腹腔镜手术联合替吉奥治疗老年进展期胃癌的疗效分析

摘　　要：目的探讨腹腔镜手术联合替吉奥治疗老年进展期胃癌的临床疗效。方法选取我院收治的老年进展期胃癌患者128例,采用随机数字表法分为两组。对照组患者采用腹腔镜手术治疗,观察组患者采用腹腔镜手术联合替吉奥进行治疗。对比两组患者的满意度、生活质量、营养指标和腹部症状。结果治疗后观察组患者满意度明显优于对照组(P<0.05);治疗后两组患者腹部症状均得到了有效的改善,其中观察组患者腹部症状改善程度明显优于对照组(P<0.05);治疗前两组患者各项生活质量指标评分均无明显差异(P>0.05),治疗后观察组患者认知功能、角色功能、躯体功能以及社会功能评分均明显高于对照组(P<0.05);治疗1 d后,两组患者的各项营养指标均无明显差异(P>0.05),治疗7 d后,观察组患者上臂三头肌肌围和血清前白蛋白水平均显著高于对照组(P<0.05),治疗14 d后,观察组患者的各项营养指标均显著优于对照组(P<0.05)。结论老年进展期胃癌患者采用腹腔镜手术联合替吉奥治疗的疗效较好。Objective To investigate the clinical efficacy of laparoscopic surgery combined with S-1 in the treatment of elderly patients with advanced gastric cancer and its effect on the occurrence of complications.Methods A total of 128 elderly patients with advanced gastric cancer were selected in our hospital.The patients were randomly divided into two groups by digital table method.The patients in the control group were treated with laparoscopic surgery.And patients in the observation group were treated by laparoscopy combined with S-1.The satisfaction degree,quality of life scores,nutritional indicators and abdominal symptoms of patients were observed and compared between the two groups.Results After treatment,patients in the observation group showed higher satisfaction degree than those in the control group(P<0.05).The abdominal symptoms of the two groups were improved effectively after the treatment,and the improvement was obviously greater in observation group than in control group(P<0.05).Before the treatment,the scores of the quality of life of the patients had no obvious difference.But after treatment,the scores of cognitive function,role function,body function and social function of the observation group were significantly higher than those of the control group(P<0.05).After 1 d treatment,there was no significant difference in the nutritional indexes between the two groups(P>0.05).After 7 d treatment,the upper arm triceps muscle circumference and the prealbumin level were found higher in observation group than in control group(P<0.05).After 14 d treatment,the nutritional indexes of the patients were all significantly higher in observation group than in control group,and the differences were significantly statistical(P<0.05).Conclusion Laparoscopic surgery combined with S-1 had fairly good effects in the treatment of advanced gastric cancer.
