清肺养阴方对肺癌放疗患者炎性因子的影响及放射性肺炎的防治效果

摘　　要：目的探讨清肺养阴方对肺癌放疗患者肿瘤坏死因子-α(TNF-α)、转化生长因子-β(TGF-β)和白介素-6(IL-6)的影响及放射性肺炎的防治效果。方法将60例肺癌患者随机分为两组,对照组30例患者采用常规放射治疗,观察组30例患者采用放射治疗联合清肺养阴方治疗。观察两组患者放射性肺炎的发生情况。放疗前、放疗过程中每周及放疗后3个月分别采血冻存,酶联免疫吸附试验(ELISA)法检测两组患者血清TNF-α、TGF-β和IL-6水平。结果观察组放射性肺炎发生率为23.3%,对照组为33.3%,差异无统计学意义(P>0.05)。观察组Ⅲ-Ⅳ级肺炎发生率为10%,对照组为26.7%,差异有统计学意义(P<0.05)。观察组患者放疗过程中及放疗后3个月血清TNF-α、TGF-β和IL-6水平均显著低于对照组,差异有统计学意义(P<0.05)。结论清肺养阴方可下调肺癌患者TNF-α、TGF-β和IL-6表达,降低Ⅲ-Ⅳ级放射性肺炎的发生率。Objective To investigate the relationship between Qingfei Yangyin formula and the changes of plasma in tumor necrosis factor-α(TNF-α),transforming growth factor-β(TGF-β)and interleukin-6(IL-6)of lung cancer patients with radiotherapy,and to evaluate its therapeutic efficacy in preventing from radiation pneumonitis(RP).Methods Sixty cases of lung cancers were divided into two groups randomly.30 cases in the observation group were treated with Qingfei Yangyin formula combined with radiotherapy.Other 30 cases in the control group were treated with conventional radiotherapy.Observe the incidence of radiation pneumonitis.Blood specimens were taken from veins before radiation,at each week during radiation and up to three months after radiation.Enzyme linked immunosorbent assay(ELISA)method was used to check the expression of TNF-α,TGF-βand IL-6 in serum.Results The RP incidence of the observation group was 23.3%,and that of the control group was 33.3%,with no statistical differences between the two groups(P>0.05).However,the incidence of gradeⅢ-ⅣRP was 10%in the observation group,significantly lower than that of the control group(26.7%)(P<0.05).During radiation and after radiation,the levels of TNF-α,TGF-βand IL-6 in the observation group were significantly lower than those of the control group(P<0.05).Conclusion Qingfei Yangyin formula could down-regulate the expressions of TNF-α,TGF-βand IL-6.It is effective in reducing the incidence of gradeⅢ-Ⅳradiation pneumonitis.
