摘  要:目的探讨血清透明质酸(HA)、IV型胶原(IV-C)、单胺氧化酶(MAO)及转化生长因子β1(TGF-β1)联合检测对肝纤维化的诊断价值。方法我院收治的120例慢性乙型肝炎患者,检测血清HA、IV-C、MAO及TGF-β1水平,并行肝组织活检,根据Metavir评分系统评价纤维化程度分期(F0~F4),比较不同肝纤维化分期患者血清HA、IV-C、MAO及TGF-β1水平,分析血清学指标与肝纤维化分期之间的相关性,采用ROC曲线分析血清学指标诊断肝纤维化的价值。结果肝组织活检纤维化程度分期显示,F0期26例,F1期19例,F2期21例,F3期24例,F4期30例;不同肝纤维化分期患者血清HA、IV-C、MAO及TGF-β1水平比较,差异均有统计学意义(P<0.05);HA、IV-C、MAO及TGF-β1均与肝纤维分期显著正相关(P<0.05);联合诊断AUC可达0.84,高于HA、IV-C、MAO及TGF-β1单独诊断(P<0.05)。结论血清HA、IV-C、MAO及TGF-β1均与肝纤维化程度密切相关,其中以TGF-β1相关性最高。HA、IV-C、MAO及TGF-β1联合应用对肝纤维化的诊断价值更高,可作为一种简单、无创、高效的方法指导肝纤维化的早期诊断。Objective To investigate the diagnostic value of combined detection of serum hyaluronic acid(HA),type IV collagen(IV-C),monoamine oxidase(MAO)and transforming growth factorβ1(TGF-β1)for liver fibrosis.Methods One hundred and twenty patients with chronic hepatitis B who were admitted to our hospital were enrolled in the study.Their fasting venous blood was collected in the morning to detect levels of serum HA,IV-C,MAO and TGF-β1.Liver biopsy was performed.The stage of fibrosis(F0~F4)was evaluated according to the Metavir scoring system,and levels of serum HA,IV-C,MAO and TGF-β1 were compared between patients with different stages of liver fibrosis.The correlation between the serological indicators and the stage of liver fibrosis was analyzed.The receiver operating characteristic curve(ROC)was drawn to analyze the value of serological markers in the diagnosis of liver fibrosis.Results The staging of fibrosis by liver biopsy showed that there were 26 cases in F0 stage,19 cases in F1 stage,21 cases in F2 stage,24 cases in F3 stage and 30 cases in F4 stage.There were statistically significant differences in levels of serum HA,IV-C,MAO and TGF-β1 among patients with different stages of liver fibrosis(P<0.05).Serum HA,IV-C,MAO and TGF-β1 were significantly positively correlated with the stage of liver fiber.ROC curve analysis showed that the AUC of combined diagnosis was 0.84,higher than that of HA,IV-C,MAO or TGF-β1 alone.Conclusion Serum HA,IV-C,MAO and TGF-β1 are closely related to the degree of liver fibrosis.Of which,TGF-β1 is the highest.The combined detection of HA,IV-C,MAO and TGF-β1 has high diagnostic value for liver fibrosis,and it can be used as a simple,non-invasive and efficient method for early diagnosis of liver fibrosis.
