摘　　要：目的探讨全视网膜光凝术(PRP)联合康柏西普玻璃体腔注射对糖尿病视网膜病变(DR)患者房水中血管内皮生长因子(VEGF)、基质细胞衍生因子1(SDF-1)表达的影响。方法我院DR患者59例,按照治疗方法不同分为光凝组(29例)和联合组(30例),光凝组给予全视网膜光凝治疗,联合组采用全视网膜光凝术联合康柏西普玻璃体腔注射治疗。观察两组房水VEGF、SDF-1水平及最佳纠正视力(BCVA)、黄斑区视网膜厚度(CMT),并发症情况。结果治疗后两组房水中VEGF、SDF-1均低于治疗前,且联合组低于光凝组(P<0.05);治疗后1、3月两组CMT、BCVA均低于治疗前,且联合组低于光凝组(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论全视网膜光凝术联合康柏西普玻璃体腔注射可有效降低DR患者房水中VEGF、SDF-1的表达,有利于提高患者治疗疗效,且安全性好,值得临床推广。Objective To investigate the effect of total retinal photocoagulation(PRP)combined with conbercept vitreous injection on the expression of vascular endothelial growth factor(VEGF)and stromal cell-derived factor 1(SDF-1)in aqueous humor of patients with diabetic retinopathy(DR).MethodS Fifty-nine DR patients in our hospital were divided into photocoagulation group(non-proliferative stage,n=29)and combined group(proliferative phase,n=30)according to the stage of DR.The photocoagulation group was treated with total retinal photocoagulation while the combined group was treated with total retinal photocoagulation combined with conbercept vitreous cavity injection.The levels of VEGF,SDF-1,optimal corrected visual acuity(BCVA)and retinal thickness(CMT)as well as complications were observed in the two groups.Results After treatment,VEGF and SDF-1 in aqueous humor of both groups were lower than those before treatment,and the combination group was lower than the photocoagulation group(P<0.05).After treatment of 1 month and 3 months,CMT and BCVA of the two groups were thinner and better than those before treatment,and the combined group was lower than the photocoagulation group(P<0.05).There was no statistically significant difference in the incidence of complications between the two groups(P>0.05).Conclusion Total retinal photocoagulation combined with conbercept vitreous cavity injection can effectively reduce the expression of VEGF and SDF-1 in aqueous humor of DR patients.This is beneficial to improve the therapeutic effect of patients and the therapy is safe.It is worthy of clinical promotion.
