摘　　要：目的研究医护人员和居民对双向转诊的了解情况、落实情况、就医倾向、参与情况及满意度,摸清双向转诊现状,找到其问题之所在。方法采用问卷调查608名医护人员双向转诊了解情况、落实情况、满意度和703名居民的就医倾向、参与情况、满意度。结果不同立场医护人员在对双向转诊了解方面差异有统计学意义(P<0.05),在接触双向转诊患者情况方面差异没有统计学意义(P>0.05)。不同家庭年收入、学历、医疗费用负担方式的居民就医倾向、双向转诊了解程度差异有统计学意义(P<0.05);不同家庭年收入、学历的居民下转意向差异有统计学意义(P<0.05);居民的双向转诊率较低且向下转诊情况堪忧。医护人员和居民对双向转诊现状的满意度均不高,不同立场医护人员的双向转诊满意度差异有统计学意义(P<0.05)。结论从引导居民合理就医、加强双向转诊制度的宣传和执行力度、构建各级医疗信息系统、建立各级医疗机构医护人员的激励机制、绩效评价机制等方面提出了对策建议,从双向转诊实施现状中找到了现阶段双向转诊的问题。Objective To investigate the understanding,implementation,medical seeking tendency,participation and satisfaction of medical staff and residents for dual referral,and find out the current situation and the problems of dual referral.Methods A questionnaire survey on the understanding,implementation and satisfaction of 608 medical staff s as well as the tendency,participation and satisfaction of 703 residents was performed.Results There were statistically significant difference in the understanding of dual referral among medical staff from different positions(P<0.05)but no significant difference was revealed in the contact with referral patients(P>0.05).The tendency to seek medical treatment and the understanding of dual referral among residents with different annual household income,education background,and ways of medical cost were significantly different(P<0.05).The downward transfer intention of residents with different annual income and educational background were statistically different(P<0.05).The dual referral rate of residents was low and worrying.In addition,the satisfaction of medical staff and residents on the current situation of dual referral was not high,and the difference between medical staff from different positions was statistically significant(P<0.05).Conclusion This paper puts forward some suggestions on guiding residents to seek medical treatment rationally,strengthening the publicity and implementation of dual referral system,building medical information system at all levels and establishing incentive and performance evaluation mechanism for medical staffs.The paper also found out the problems of dual referral at the present stage.
