摘　　要：为了探讨MRI对直肠癌术前TN分期的评价,本研究选取31例行直肠系膜全切术治疗的直肠癌患者进行研究。患者术前均行MRI扫描,根据肿瘤的浸润肠壁深度、范围和淋巴结转移来确定TN分期,考察直肠系膜筋膜受累情况,并测量低位直肠癌患者肿瘤至肛缘的距离。研究显示,T分期总准确率为74.1%,Kappa值为0.630,N分期正确率为67.7%,κappa值为0.509。MRI判定直肠系膜筋膜受累情况正确率为90.3%,κappa值为0.784。将MRI曲线测得肿瘤下缘与肛缘距离,以及直肠指诊所测得数据进行T检验,两者差异无显著性,无统计学意义(p>0.05)。本研究结果显示,MRI可以较准确的预测直肠癌的TN分期,可以为直肠癌术前评估提供重要的临床信息,是一种值得推广的临床检查方法。In order to assessment the evaluation of preoperative TN staging of rectal cancer by MRI. Thirty-one patients with rectal cancer who underwent total mesorectal excision were enrolled. All patients underwent MRI scan before operation. TN staging was determined according to the depth, extent and lymph node metastasis of the infiltrating tumor wall. The involvement of mesorectal fascia was investigated and the distance from tumor to anal margin in patients with low rectal cancer was measured. research showed the overall accuracy of T staging was74.1%, κappa value was 0.630, N staging accuracy was 67.7%, κappa value of 0.509. Mesorectal fascia MRI to determine the correct rate of involvement 90.3%, kappa value was 0.784. MRI curve will be measured with the lower edge of the tumor from the anal margin and the digital rectal clinic data measured T test, the two there was no significant difference(p>0.05). MRI can be more accurate prediction of TN staging of rectal cancer, preoperative assessment of rectal cancer can provide important clinical information, is a worthy of promotion of the inspection method.
