白内障及糖尿病性视网膜病变的联合手术观察

摘　　要：目的观察白内障及增生性糖尿病性视网膜病变(PDR)联合手术的效果。方法回顾性分析2015年2月至2018年1月该院眼科白内障伴PDR 142例(142眼)的临床资料。患者随机分为两组:采用超声乳化人工晶状体植入联合玻璃体切除术者为A组(72眼);采用玻璃体切除联合晶状体切除及人工晶状体植入者为B组(70眼)。结果两组之间术前视力差异无统计学意义(P=0.453),而术后视力差异有统计学意义(Z=-4.121,P=0.035)。术后眼压两组间各时间点比较差异均无统计学意义(P>0.05)。A组角膜水肿和晶状体囊浑浊发生率低于B组(P<0.05)。结论白内障及增生性糖尿病视网膜病变采用超声乳化联合玻璃体切除术术后视力优于晶状体切除联合玻璃体切除术,术后并发症前者少于后者。Objective To observe the efficacy of the combined surgery of cataract and proliferative diabetic retinopathy(PDR).Methods The data of 142 eyes of 142 cases of cataract with PDR from Feb.2015 to Jan.2018 in this hospital were analyzed retrospectively.All cases were randomly divided into two groups.Group A,72 eyes received phacoemulsification combined with vitrectomy and intraocular lens implantation.Group B,70 eyes received vitrectomy combined with lensectomy and intraocular lens anterior capsular ciliary groove implantation.Results Between the two groups,there was no significant difference in preoperative visual acuity(P=0.453),but the difference in postoperative visual acuity was statistically significant between the two groups(Z=-4.121,P=0.035).There was no significant difference in intraocular pressure after operation between the two groups at different time points(P>0.05).The incidence of corneal edema and lens capsule opacity in group A were lower than that in group B(P<0.05).Conclusion The visual acuity after phacoemulsification combined with vitrectomy for cataract with proliferative diabetic retinopathy is better than that of lensectomy combined with vitrectomy,and the complications are less than that of vitrectomy combined with vitrectomy.
