摘  要:目的检测乳牙慢性根尖周炎未经治疗和根管治疗失败病例根尖周病损组织的菌群构成,比较两者优势菌之间的差异。方法收集乳牙慢性根尖周病损组织未经治疗和根管治疗失败病例各30例,提取细菌基因组DNA,构建16S rDNA克隆文库并进行测序,鉴定分析致病优势菌。结果未经治疗组与根管治疗失败组鉴定出细菌或基因型分别为245种和155种,链球菌属(Streptococcus sp.)、弯曲菌属(Campylobacter sp.)和嗜血杆菌属(Haemophilus sp.)在两组均有高检出率,Haemophilus sp.和未培养菌在根管治疗失败组占比例较高,有显著差异。结论乳牙慢性根尖周病损组织是以G-厌氧及兼性厌氧菌为主的混合感染,优势菌分布存在较大个体差异。纤细弯曲菌与乳牙慢性根尖周病密切相关,Haemophilus sp.在根管治疗失败持续性感染中可能发挥重要作用。Objective To investigate the bacterial compositions in primary and persistent apical periodontitis of deciduous teeth.Methods Tissue samples were collected from 60 periapical lesions in deciduous teeth with primary and persistent apical periodontitis and detected for their bacterial composition using 16S rDNA cloning and sequencing.Results A total of 245 and 155 species or genotypes were identified in primary and persistent apical periodontitis of deciduous teeth.Sequencing revealed a high prevalence of Streptococcus sp.,Campylobacter sp.and Haemophilus sp.in both types of infections.More uncultured phylotypes and Haemophilus sp.were detected in persistent infections.Conclusion Chronic periapical lesions in deciduous teeth contained polymicrobial infections.The distribution of dominant bacteria in two groups showed siginificant difference.The bacterial load and microbial profile of primary infections were significantly different from persistent infections,which indicated the presence of diverse bacterial populations in periapical lesions.
