螺旋CT与数字胃肠造影技术对胃癌患者的诊断价值

摘　　要：目的:分析螺旋CT技术与数字胃肠造影对胃癌患者进行检查的应用价值。方法:采用数表法随机选取医院收治的73例胃癌患者资料进行回顾性研究,所有患者术前分别使用螺旋CT技术与数字胃肠造影技术进行检查,观察比较两种检查结果的早期癌灶检出、癌灶定性及病灶定位等差异,比较数字胃肠造影检查与病理诊断结果间的差异,比较两种方法在检查胃癌不同分型中的准确率以及影像学表现。结果:73例胃癌患者发病部位中广泛浸润皮革胃4例,胃体15例,胃底贲门部25例,胃窦部29例。两种方检查法早期癌灶检出、癌灶定性、病灶定位等比较差异无统计学意义。术前数字胃肠造影检查准确率Ⅰ型为81.82%(9/11),Ⅱ型为78.72%(37/47),Ⅲ型为86.67%(13/15),总检查准确率为80.82%(59/73);螺旋CT检查准确率Ⅰ型为72.73%(8/11),Ⅱ型为85.11%(40/47),Ⅲ型为93.33%(14/15),总检查准确率为84.93%(62/73);螺旋CT与胃肠造影检查在胃癌不同分型中的准确率比较差异无统计学意义。结论:术前两种方法检查胃癌均具有一定价值,数字胃肠造影可清晰显示胃腔内变化;螺旋CT能够准确评估手术适应症与胃腔外远处器官转移情况,故合理选择诊断方式能够有效提升术前胃癌的诊断与鉴别效果。Objective:To observe and analyze the application value of spiral CT and digital gastrointestinal angiography in the examination for patients with gastric cancer.Methods:A retrospective study was conducted on the data of 73 patients with gastric cancer who were selected as random number table.All patients underwent spiral CT and digital gastrointestinal angiography before operation.And the differences of early detection of cancer focus,qualitative diagnosis of focus and location of focus between the two kinds of results were observed and compared.And the differences of results between digital gastrointestinal angiography and pathological diagnosis were compared.And the accuracy and imaging performance of two kinds of methods in examining different classification of gastric cancer were compared.Results:In 73 patients with gastric cancer,4 cases extensively infiltrated into leather bottle stomach,15 cases infiltrated into the gastric body,25 cases infiltrated into the cardiac part of stomach fundus,and 29 cases infiltrated into the gastric antrum.The results of spiral CT and digital gastrointestinal angiography showed that there was no significant difference between the two methods in early detection,qualitative diagnosis and localization of focus.The accuracy rate of preoperatively digital gastrointestinal angiography of type I was 81.82%(9/11),that of type II was 78.72%(37/47),that of type III was 86.67%(13/15),and the total accuracy rate of detection was 80.82%(59/73).And the accuracy rate of spiral CT examination of type I was 72.73%(8/11),type II was 85.11%(40/47),type III was 93.33%(14/15),and the totally accuracy rate was 84.93%(62/73).There was no significant difference in the accuracy rate of different types of gastric cancer between spiral CT and gastrointestinal angiography.Conclusion:Both two preoperative gastric cancer examination methods have certain value.Digital gastrointestinal angiography can clearly show the changes in the gastric cavity.And spiral CT can accurately assess the adaptation disease o
