摘　　要：目的探讨血清降钙素原(PCT)及超敏C反应蛋白(hs-CRP)在脑卒中相关肺炎(SAP)的应用价值。方法选取郑州人民医院2015-01—2019-01收入重症医学科的患者。选取其中SAP患者102例及同期无伴SAP的脑卒中患者58例为对照组。比较2组患者入院时、住院7 d后的PCT、hs-CRP水平。比较SAP患者中转归良好组和转归不良组PCT、hs-CRP水平。结果SAP组与无伴SAP组血清PCT、hs-CRP比较,入院时两者差异无统计学意义,但住院7 d后,SAP组PCT、hs-CRP水平较对照组高(P<0.05)。SAP组中转归不良组入院时和住院7 d后的PCT水平均较转归良好组高(P<0.05)。2组入院时hs-CRP水平无明显差异,而住院7 d后转归不良组高于转归良好组(P<0.05)。结论PCT及hs-CRP可作为脑卒中患者筛查SAP的早期敏感指标,并可作为SAP患者病情监测指标。Objective To investigate the application value of serum procalcitonin(PCT)and hypersensitive C-reactive protein(hs-CRP)in stroke-associated pneumonia(SAP).Methods Patients admitted to the emergency intensive care unit of our hospital from January 2015 to January 2019 were selected.Among them,102 patients with SAP and 58 patients without SAP were selected as the control group.PCT and hs-CRP levels were compared at admission and 7 days after admission.The PCT and hs-CRP levels of the good group and the bad group in SAP patients were compared.Results The comparison results of serum PCT and hs-CRP between the SAP group and the unaccompanied SAP group showed that there was no statistically significant difference between the two groups at the time of admission,but the PCT and hs-CRP levels in the SAP group were higher than those in the control group after 7 days of hospitalization(P<0.05).In the SAP group,the PCT level of the group with poor outcome was higher at the time of admission and 7 days after admission than that of the group with good outcome(P<0.05).There was no significant difference in hs-CRP level between the two groups at the time of admission,but after 7 days of hospitalization,the poor group was higher than the good group(P<0.05).Conclusion PCT and hs-CRP can be used as early sensitive indicators for screening SAP in stroke patients,and can also be used as condition monitoring indicators for SAP patients.
