马来酸氟伏沙明联合普拉克索对帕金森性抑郁非运动症状的效果观察

摘　　要：目的观察马来酸氟伏沙明联合普拉克索对帕金森性抑郁患者非运动症状的改善效果。方法收集2016-01—06安阳地区医院收治的70例帕金森性抑郁病例,依据随机抽签法分为2组各35例,对照组给予普拉克索,观察组给予马来酸氟伏沙明联合普拉克索方案,比较2组治疗前后蒙特利尔认知量表(MoCA)评分、汉密尔顿抑郁(HAMD)评分、汉密尔顿焦虑(HAMA)评分、帕金森病综合评分(UPDRS)、肢体感觉异常率、用药不良反应状况。结果经过治疗,观察组MoCA量表中记忆、抽象与视空间评分均明显高于对照组(P<0.05);观察组HAMD、HAMA评分、UPDRS量表中精神行为和情绪、日常活动及运动功能评分均明显低于对照组(P<0.05);治疗期间,观察组肢体痛、不宁腿综合征、肢体痉挛以及嗅觉异常发生率均明显低于对照组(P<0.05),2组不良反应总发生率比较无显著差异(P>0.05)。结论马来酸氟伏沙明联合普拉克索治疗帕金森性抑郁患者,可促进神经认知功能恢复,减少抑郁焦虑情绪,显著改善帕金森病情,降低肢体感觉异常出现率,且不加重药物不良反应。Objective To observe the effects of fluvoxamine maleate combined with pramipexole on the non-motor symptoms in Parkinsonian patients.Methods 70 cases of Parkinson depression patients collected in our hospital from January 2016 to June 2016 were included in the study and divided into the 2 groups with 35 cases in each group according to the random lottery method.The control group was given pramipexole,and the observation group was given the regimen of fluvoxamine maleate combined with pramipexole.The Montreal cognitive assessment(MoCA) score,Hamilton depression score(HAMD),Hamilton anxiety(HAMA) score,unified Parkinson disease rating scale(UPDRS) score,acroparesthesia rate,adverse drug reaction condition were compare between the 2 groups before and after treatment.Results After treatment,the scores of memory,abstraction and visual space in the MoCA scale were significantly higher in the observation group than those in the control group(P<0.05).The scores of HAMD and HAMA,and the scores of mental,behavioral and emotional,daily activities and motor function of UPDRS scale were significantly lower in the observation group than those in the control group(P<0.05).During treatment,the incidence rates of limb pain,restless legs syndrome,limb spasticity and olfactory abnormality were significantly lower in the observation group than those in the control group(P<0.05),and there was no significant difference in the incidence rate of adverse reactions between the 2 groups(P>0.05).Conclusion For Parkinson depression patients,fluvoxamine maleate combined with pramipexole drug regimen can promote the neurological cognitive function recovery,reduce depression and anxiety,and significantly improve the condition of Parkinson’s disease,and reduce the acroparesthesia occurrence without increasing the adverse drug reaction.
