摘　　要：目的:探讨胃癌患者血清淀粉样蛋白A含量在胃癌诊断中的应用价值。方法:选取在本院收治的120例胃癌患者和30例健康体检者血清为实验对象,采用ELISA法检测血清中SAA1的表达水平,分析其与临床病理因素的相关性。结果:胃癌患者血清中SAA1表达含量显著高于健康体检者,差异具有统计学意义(t=-9.725,P<0.001)。ROC曲线分析显示,SAA1诊断AUC为0.907(95%CI:0.838~0.976),检测阈值为29.02μg/mL,其诊断敏感性和特异性分别为93.3%和86.7%。患者SAA1的表达水平与胃癌患者的性别(r=0.056,P>0.05)、年龄(r=-0.032,P>0.05)、肿瘤转移(r=-0.173,P>0.05)情况无明显相关,与肿瘤的大小(r=-0.228,P<0.05)和侵袭程度(r=-0.409,P<0.05)具有相关性,且随着肿瘤侵袭程度的增加而增加。结论:胃癌患者血清SAA1表达水平显著升高,可作为胃癌诊断的新型潜在标志物。Objective:To explore the value of the content of serum amyloid A in the diagnosis of gastric cancer in patients with gastric cancer.Methods:The serum of 120 gastric cancer patients and 30 healthy physical examination patients admitted to Hainan Provincial Hospital of Traditional Chinese Medicine from September 2015 to December 2017 were selected as experimental subjects.Results:The expression of SAA1 in serum of patients with gastric cancer was significantly higher than healthy subjects(42.38±10.12)μg/mL vs(22.70±8.98)μg/mL,and the difference was statistically significant(t=-9.725,P<0.001).ROC curve analysis showed that the AUC of SAA1 diagnosis of gastric cancer was 0.907(95%CI:0.838~0.976),the detection threshold was 29.02μg/mL,and the diagnostic sensitivity and specificity were 93.3%and 86.7%,respectively.The expression level of serum SAA1 was not correlated with gender(r=0.056,P>0.05),age(r=-0.032,P>0.05)and tumor metastasis(r=-0.173,P>0.05)of gastric cancer patients,and correlated with tumor size(r=-0.228,P<0.05)and invasion degree(r=-0.409,P<0.05),and increased with the increase of tumor invasion degree.Conclusion:The expression level of serum SAA1 in gastric cancer patients is significantly increased,which may be a new potential marker for the diagnosis of gastric cancer.
