MCST检测AIP及胰腺癌的影像学特征及诊断价值分析

摘　　要：目的:探讨多层螺旋CT(MSCT)对自身免疫性胰腺炎(AIP)及胰腺癌的鉴别诊断价值。方法:选取2014年1月—2018年6月山东大学齐鲁医院收治的40例胰腺癌、67例AIP患者的影像资料,对比两组的影像学特征,并以病理学结果作为诊断金标准,评价MSCT诊断胰腺癌和AIP的临床价值。结果:MSCT检查显示,AIP患者和胰腺癌患者的胰腺形态、胰腺病灶密度、胰腺病灶平扫期密度、胰腺病灶门脉期密度情况比较,差异均具有统计学意义(P<0.05);AIP和胰腺癌患者病灶动脉期密度比较,差异均不具有统计学意义(P>0.05);MSCT诊断AIP患者和胰腺癌的灵敏度为90.00%、特异度为85.07%、漏诊率为10.00%、误诊率为14.93%。结论:AIP及胰腺癌患者在MSCT下具有不同的影像学特征,MSCT对于二者的鉴别诊断具有较高的临床实用价值。Objective:To explore the value of multi-slice spiral CT(MSCT)in differential diagnosis of autoimmune pancreatitis(AIP)and pancreatic cancer.Methods:Forty patients with pancreatic cancer and 67 patients with AIP(January 2014 to June 2018)in our hospital were selected as the research objects.Both groups were examined by MSCT.The imaging characteristics of the two groups were compared.The clinical value of MSCT in differential diagnosis of pancreatic cancer and AIP was calculated by using pathological results as the diagnostic gold standard.Results:The results of MSCT in AIP patients and pancreatic cancer patients showed that there were significant differences in pancreatic morphology,pancreatic lesion density,pancreatic lesion plain scan density and portal phase density between AIP patients and pancreatic cancer patients(P<0.05),but there was no significant difference in arterial phase density between AIP patients and pancreatic cancer patients(P>0.05).The sensitivity,specificity,missed diagnosis and misdiagnosis of pancreatic cancer were 90.00%,85.07%,10.00%and 14.93%,respectively.Conclusion:AIP and pancreatic cancer patients have different imaging characteristics under MSCT.MSCT has high clinical value in differential diagnosis of AIP and pancreatic cancer.
