急性胰腺炎患者血清IL-6、IL-10水平变化及其与淋巴细胞自噬凋亡的相关性

摘　　要：目的观察急性胰腺炎(AP)患者血清白细胞介素6(IL-6)、IL-10水平变化,探讨其与淋巴细胞自噬、凋亡的关系.方法73例AP患者中轻症急性胰腺炎(MAP,MAP组)25例、中度重症急性胰腺炎(MSAP,MSAP组)25例、重症急性胰腺炎(SAP,SAP组)23例,选择同期健康体检者25例为对照组.AP患者入院24 h内采集外周血,对照组体检时采集外周血,ELISA法检测各组血清IL-6、IL-10、细胞自噬蛋白微管相关蛋白1轻链3(LC3)及凋亡相关因子半胱氨酸天冬氨酸特异性蛋白酶3(Caspase-3),流式细胞术测算各组淋巴细胞自噬率、凋亡率,Pearson相关分析法分析AP患者血清IL-6、IL-10水平与血清LC3、Caspase-3水平的相关性.结果①血清IL-6、IL-10水平:与对照组比较,MAP、MSAP、SAP组血清IL-6、IL-10水平升高(P均<0.05);与MAP组比较,MSAP组血清IL-6水平升高、IL-10水平降低(P均<0.05);与MSAP、MAP组比较,SAP组血清IL-6水平升高、IL-10水平降低(P均<0.05).②淋巴细胞自噬情况:与MSAP、MAP及对照组比较,SAP组血清LC3水平、淋巴细胞自噬率升高(P均<0.05);与MAP组、对照组比较,MSAP组血清LC3水平、淋巴细胞自噬率升高(P均<0.05).③淋巴细胞凋亡情况:与MSAP、MAP及对照组比较,SAP组血清Caspase-3水平、淋巴细胞凋亡率升高(P均<0.05);与MAP组及对照组比较,MSAP组血清Caspase-3水平、淋巴细胞凋亡率升高(P均<0.05).④AP患者血清IL-6、IL-10水平与血清LC3、Caspase-3水平的相关性:MSAP、SAP组患者血清IL-6水平与血清LC3、Caspase-3水平呈正相关(r分别为0.9904,0.9813,P均<0.05;r分别为0.9785,0.9870,P均<0.05),血清IL-10水平与血清LC3、Caspase-3水平呈正相关(r分别为0.9331,0.9732,P均<0.05;r=0.9878,0.8663,P均<0.05).结论AP患者血清IL-6、IL-10、LC3、Caspase-3水平均升高,MSAP、SAP患者淋巴细胞自噬、凋亡水平均升高.血清IL-6、IL-10水平与血清LC3、Caspase-3水平呈正相关.血清IL-6、IL-10可能通过促进AP患者淋巴细胞的自Objective To observe the changes in levels of serum interleukin 6(IL-6)and IL-10 in the peripheral blood of patients with acute pancreatitis(AP),and to discuss its relationship with autophagy and apoptosis of lymphocytes.Methods Seventy-three patients with AP were divided into three groups,including 25 cases with mild acute pancreatitis(MAP,MAP group),25 cases with moderately severe acute pancreatitis(MSAP,MSAP group),and 23 cases with severe acute pancreatitis(SAP,SAP group).Meanwhile,25 healthy volunteers were selected as the control group.Peripheral blood of patients with AP was collected within 24 h,and the peripheral blood of control group was collected during physical examination.The levels of IL-6,IL-10,microtubule-associated protein 1 light chain 3(LC3)and Caspase-3 in peripheral blood of patients with AP were determined by enzyme-linked immunosorbent assay(ELISA).Flow cytometry was used to measure the rate of autophagy and apoptosis of lymphocytes in each group.Pearson correlation analysis was used to analyze the correlation of serum IL-6 and IL-10 levels with serum LC3 and Caspase-3 levels in patients with AP.Results(1)The levels of serum IL-6 and IL-10:compared with the control group,the levels of serum IL-6 and IL-10 in the MAP,MSAP,and SAP groups increased(all P<0.05).Compared with the MAP group,the serum IL-6 level in the MSAP group increased,while the level of serum IL-10 decreased(all P<0.05).Compared with the MSAP group and MAP group,the serum IL-6 level in the SAP group increased,while the serum IL-10 level decreased(all P<0.05).(2)The rate of lymphocyte autophagy:compared with the MSAP,MAP,and control groups,the serum LC3 level and rate of lymphocyte autophagy in the SAP group increased(all P<0.05).Compared with the MAP group and control group,the serum LC3 level and rate of lymphocyte autophagy in the MSAP group increased(all P<0.05).(3)The rate of lymphocyte apoptosis:compared with the MSAP,MAP,and control groups,the level of serum Caspase-3 and rate of lymphocyte apoptosis in the SAP group i
