我国187家医院ICU护士谵妄知识掌握与谵妄管理开展现况的调查分析

摘　　要：目的调查ICU护士谵妄知识掌握情况及其所在医院谵妄管理的现状,分析其影响因素,以期为提高ICU护士对谵妄的识别及管理能力提供依据。方法采用便利抽样法,应用自行设计的ICU谵妄现状资料收集表对来自全国20个省、自治区、直辖市187家医院的中华护理学会2018年重症专科护士培训班学员进行问卷调查。统计护士ICU谵妄知识掌握情况并分析其影响因素,调查护士所在医院ICU谵妄管理情况。结果ICU护士对ICU谵妄的定义、临床表现、危险因素、评估工具、治疗、护理及预防知识的掌握程度得分3(3,4)分(5分制);参加和没有参加过系统的ICU谵妄相关培训的分别有94、93名,两组护士对谵妄知识掌握情况比较,差异有统计学意义(P<0.01);132名护士所在医院尚未开展ICU谵妄评估;仅14名护士所在医院实施ICU谵妄的集束化管理措施。结论ICU护士对ICU谵妄的相关知识掌握不足,ICU谵妄管理的临床实施率低,应通过系统培训提高ICU护士知识水平,进而改变其对ICU谵妄管理的态度和行为。Objective To investigate the status of delirium knowledge and delirium management among intensive care unit(ICU)nurses,and analyze the influencing factors,so as to provide evidence for improving the recognition and management ability of delirium among ICU nurses.Methods By convenience sampling method and self-designed ICU Delirium Status Data Collection Table,a questionnaire survey was conducted among participants in the 2018 critical care specialist nurses training course held by the Chinese Nursing Association of 187 hospitals from 20 provinces,autonomous regions and municipalities directly under the Central Government.The knowledge of delirium among nurses in ICU and the influencing factors were analyzed.The ICU delirium management in the participating nurses'hospitals were investigated.Results ICU nurses'score in mastery of the definition,clinical manifestation,risk factors,assessment tools,treatment,nursing and prevention knowledge of delirium in ICU was 3(3,4)points(5 points system);94 nurses had participated in and 93 nurses had never participated in systematic training of ICU delirium.There were statistical differences in the mastery of delirium knowledge between the two groups(P<0.01);132 nurses'hospitals had not yet carried out ICU delirium assessment;only 14 nurses'hospitals implemented ICU delirium cluster management measures.Conclusions ICU nurses have insufficient knowledge of ICU delirium,and the clinical implementation rate of delirium management in ICU is low.Systematic training should be carried out to improve the knowledge level of ICU nurses in order to change their attitude and behavior towards delirium management in ICU.
