摘 要:目的探讨X线摄影对微钙化型乳腺癌病变范围测量的准确性,进一步分析其影响因素。资料与方法回顾性分析167例X线摄影检查表现为单纯微钙化的乳腺癌患者的病例资料。按照X线测量值将所有患者分为A组(X线测量值≤2 cm)和B组(X线测量值>2 cm)。分析X线摄影与病理测量病变范围的相关性和影响X线摄影测量准确性的因素。结果Spearman相关分析显示,X线摄影与病理所测病变范围呈中度相关(rho=0.478,P<0.001)。X线摄影准确评估病变范围82例(49.1%)、高估病变77例(46.1%)、低估病变8例(4.8%)。多因素Logistic回归分析提示,X线测量值是X线摄影测量微钙化型乳腺癌病变范围准确性的独立预测因素(OR=16.69,95%CI 4.55~61.28,P<0.001)。结论X线摄影易高估微钙化型乳腺癌病变范围,其评估准确性与X线测量值相关。Purpose To discuss the accuracy of mammography in measuring the scope of microcalcified breast cancer lesions and further analyze its influencing factors.Materials and Methods Retrospective analysis was conducted for the data of 167 patients with simple microcalcified breast cancer showed by X-ray examination results.According to X-ray measurement values,all cases were divided into group A(X-ray measurement value≤2 cm)and group B(X-ray measurement value>2 cm).The correlation between mammography and lesion scope by pathological measurement and the factors that affected the accuracy of mammography measurement were analzyed.Results Spearman correlation analysis showed that mammography was moderately correlated with lesion scope by pathological measurement(rho=0.478,P<0.001).Mammography accurately estimated lesion scope for 82 cases(49.1%),overestimated lesion for 77 cases(46.1%),and underestimated lesion for 8 cases(4.8%).Multivariate Logistic regression analysis showed that the value measured by X-ray was the independent predictor that was associated with the accuracy of mammography in measuring the scope of microcalcified breast cancer lesions(OR=16.69,95%CI 4.55-61.28,P<0.001).Conclusion Mammography tends to overestimate the scope of microcalcified breast cancer lesions and its evaluation accuracy is related to the value measured by X-ray.
