摘 要:目的验证血清淀粉样P物质(SAP)在正常孕妇和有神经管缺陷患儿的孕妇血清中的表达情况,探讨其作为神经管缺陷产前诊断标志物的可能。方法于清晨空腹抽取病例组(12例胎儿患神经管缺陷的孕妇)和对照组(12例胎儿正常的孕妇)外周静脉血5 mL后静置离心,抽取上层血清,用Western blotting方法检测SAP表达水平,并计算受试者操作特征(ROC)曲线,评估SAP诊断神经管缺陷的价值。结果在妊娠12~19周、21~28周时,病例组孕妇血清中SAP表达明显下调,差异有统计学意义(P<0.05)。ROC曲线下面积(AUC)=0.770 8(95%CI:0.573 9~0.967 8,P<0.05)。当敏感度为83.33%时,其特异度为66.67%。结论胎儿神经管缺陷的孕妇血清中SAP表达显著降低,提示SAP有作为神经管缺陷的产前诊断标志物的潜在可能。Objective To verify the expression of serum amyloid P component(SAP)in healthy pregnant women and pregnant women with fetal neural tube defects(NTDs),and to explore its feasibility as a prenatal biomarker for the diagnosis of NTDs.Methods Twelve pregnant women with fetal NTDs were selected as the case group;and 12 healthy pregnant women,as the control group.Peripheral venous blood was extracted from the pregnant women early in the morning.After centrifugation,the upper serum was obtained.The SAP expression level was measured using Western blotting,and a receiver-operating characteristic curve(ROC)was obtained to evaluate the diagnostic value of SAP level.Results During 12-19 and 21-28 weeks of pregnancy,the SAP expression level in the pregnant women with fetal NTDs was downregulated,and the difference was statistically significant(P<0.05).The area under the ROC curve was 0.770 8(95%confidence interval:0.573 9-0.967 8,P=0.024).When the sensitivity was 83.33%,the specificity was 66.67%.Conclusion The SAP expression levels of the pregnant women with fetal NTDs were significantly reduced.This suggests that SAP expression level could be a potential biomarker for the prenatal diagnosis of NTDs.
