摘  要:目的探讨实验性脑出血(ICH)大鼠脑组织中组织蛋白酶L(Cat L)的表达及其病理意义。方法随机选取成年雄性Wistar大鼠90只,采集自体股动脉血(100μL)注射至大鼠右侧基底节制作ICH模型大鼠,另取45只大鼠(假手术组)注入100μL生理盐水。将ICH大鼠随机分为ICH组、选择性Cat L抑制剂(Z-FY-CHO)组。建模成功后2 h Z-FY-CHO组侧脑室注射二甲基亚砜(DMSO)溶解的Z-FY-CHO 2μL(1μg/μL),ICH组侧脑室注射等量DMSO溶剂。术后第3天检测各组大鼠血肿周围脑组织Cat L mRNA表达,术后第1、3、7、14天各组进行神经功能评分;采用干湿重法检测脑含水量;注射依文思蓝检测血脑屏障通透性。结果与假手术组比较,ICH组大鼠血肿周围脑组织中Cat L mRNA表达上调(P<0.05);与ICH模型组比较,Z-FY-CHO组大鼠脑组织中Cat L mRNA表达下调,大鼠神经损伤程度明显减轻,脑含水量明显降低,血脑屏障破坏程度减轻,差异均有统计学意义(均P<0.05)。结论实验性ICH大鼠脑组织中Cat L mRNA表达上调,Cat L抑制剂Z-FY-CHO能够抑制Cat L mRNA表达,减轻神经损伤程度,提示Cat L表达与ICH大鼠神经损伤相关。Objective We studied the expression and pathological significance of cathepsin L(Cat L)in the brain tissue of rats with intracerebral hemorrhage(ICH).Methods Adult male SPF Wistar rats(n=135)were randomly selected to either receive a stereotactic injection of 100μL autologous arterial blood(n=90)or sterile saline(n=45)into the right basal ganglia.The rats who were injected with blood were then randomly allocated to one of two groups:Z-FY-CHO or ICH.Rats in the Z-FY-CHO group were treated with Z-FY-CHO dissolved in dimethyl sulfoxide(DMSO;2μL,1μg/μL)intracerebroventricularly,2 hours after blood injection.Rats in the ICH group were injected with the same volume of DMSO.The expression of Cat L in the rat brains was detected using real-time quantitative polymerase chain reaction(PCR)3 days after ICH.The brain water content,neurologic impairment scores,and Evans blue content were assessed at 1,3,7,and 14 days after ICH.Results Compared with rats in the sham group,those in the ICH group had higher Cat L mRNA expression(P<0.05).Rats in the Z-FY-CHO group had a lower Cat L mRNA expression than rats in the ICH group(P<0.05).Moreover,rats in the Z-FY-CHO group also had lower neurologic impairment scores,brain water content,and Evans blue content(P<0.05).Conclusion Brain Cat L mRNA expression increased after experimental ICH in rats.Z-FY-CHO can inhibit the expression of Cat L and alleviate brain injury.Thus,Cat L expression appears closely related to post-ICH injury in rats.
