广泛肿块切除术与乳腺局部肿块切除术治疗乳腺叶状肿瘤的效果比较

摘　　要：目的探讨广泛肿块切除术与乳腺局部肿块切除术治疗乳腺叶状肿瘤的效果。方法选取2011年1月~2013年5月我院收治的100例乳腺分叶状肿瘤患者作为研究对象。根据手术方法的不同将其分为观察组和对照组,每组各50例。观察组患者采用广泛肿块切除术治疗,对照组患者采用乳腺局部肿块切除术治疗。比较两组患者术后1年的患侧肢体功能(前屈度、后伸度、内旋度、外展度、外旋度、摸高高度)、术后5年的疾病复发率及生存率等指标。结果观察组患者患侧肢体的前屈度、后伸度、内旋度与对照组比较,差异无统计学意义(P>0.05);观察组患者患侧肢体的外展度、外旋度及摸高高度均高于对照组,差异有统计学意义(P<0.05)。观察组患者术后5年的疾病复发率(12.0%)与对照组(14.0%)比较,差异无统计学意义(P>0.05);观察组患者术后5年的生存率(12.0%)与对照组(10.0%)比较,差异无统计学意义(P>0.05)。结论在乳腺分叶状肿瘤的治疗中,广泛肿块切除术较乳腺局部肿块切除术的预后更好,值得临床推广应用。Objective To explore the effect between extensive lumpectomy and local lumpectomy for phyllodes tumor of breast.Methods A total of 100 patients with phyllodes tumor of breast admitted to our hospital from January 2011 to May 2013 were selected as the research objects.According to different surgical methods,they were divided into observation group and control group,50 cases in each group.Patients in the observation group were treated with extensive lumpectomy,while patients in the control group were treated with local lumpectomy.The limb function(flexion,extension,internal rotation,abduction,external rotation,touch height)of the affected side 1 year after operation,disease recurrence rate and survival rate 5 years after operation were compared between the two groups.Results There were no significant difference in muscle strength,flexion,extension and pronation between the observation group and the control group(P>0.05).The abduction degree,abduction degree and touch height of the affected limb in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The disease recurrence rate in the observation group(12.0%)was compared with that in the control group(14.0%)at 5 years after operation,and the difference was not statistically significant(P>0.05).The 5-year survival rate of the observation group(12.0%)was not statistically significant compared with that of the control group(10.0%)(P>0.05).Conclusion In the treatment of breast phyllodes tumor,extensive lumpectomy has a better prognosis than local lumpectomy,which is worthy of clinical application.
