摘　　要：目的探讨嗜酸细胞性胃肠炎(EG)的临床特点及诊治方法。方法回顾性分析2017年1月至2018年12月首都医科大学附属北京友谊医院收治的11例EG患者的临床资料,观察其临床表现、实验室检查、影像学特征、内镜特征、病理特征及预后,采用电话、门诊及住院复查进行随访,随访截至2019年4月。结果EG临床表现缺乏特异性,可表现为腹痛、腹泻、便血等症状。外周血及骨髓嗜酸性粒细胞(EOS)可明显升高。影像学检查缺乏特异性表现。内镜检查表现为多发黏膜充血、水肿,病理可见大量EOS浸润。所有患者经激素治疗后症状均得到缓解,复发病例再次应用激素治疗有效。结论EG临床表现、影像学检查、内镜检查均缺乏特异性,外周血、骨髓EOS升高及内镜病理有助于诊断,激素治疗预后良好。Objective To explore clinical characteristics,diagnosis and treatment method of eosinophilic gastroenteritis.Methods Eleven cases with eosinophilic gastroenteritis from January 2017 to December 2018 were analyzed retrospectively in Beijing Friendship Hospital,Capital Medical University.The patients were followed up by phone,outpatient service,and hospitalization.The clinical features,laboratory examination,imaging features,digestive endoscopy features,pathological features and prognosis were observed.The follow-up deadline was April 2019.Results The clinical characteristics of EG were not specific,including abdominal pain,diarrhea and bloody stool etc.EOS increased obviously in blood and bone marrow.Imaging features were not specific.Digestive endoscopy could find multiple congestion and edema in gatrointestinal mucosa,and a large number of EOS could be found in biopsy pathology.The symptoms of eleven cases were relieved under the therapy with glucocorticoid.Reusing of glucocorticoid is effective in recurrence cases.Conclusion EG is not specific in clinical features,imaging features and digestive endoscopy features,EOS in blood and bone marrow increased in blood and bone marrow,and digestive endoscopy biopsy pathology are helpful in EG diagnosis.Glucocorticoid therapy has a good prognosis.
