替吉奥联合同步推量调强放疗对老年局部晚期鼻咽癌疗效和免疫功能的影响

摘　　要：目的探讨替吉奥联合同步推量调强放疗对老年局部晚期鼻咽癌的疗效和免疫功能影响,为治疗鼻咽癌提供方法。方法选取2015年4月—2017年4月来我院就诊的老年局部晚期鼻咽癌患者102例,采用随机数字表法分为观察组(n=54)和对照组(n=48)。观察组患者给予替吉奥联合同步推量调强放疗,对照组患者给予常规调强放疗。评估并比较2组患者的临床疗效、不良反应发生率和免疫功能等情况。结果治疗3个月,观察组治疗有效率显著高于对照组(P<0.05)。观察组皮肤损伤发生率、黏膜损伤发生率、唾液腺损伤发生率和中耳炎发生率均显著低于对照组(P<0.05)。2组1年生存率差异无统计学意义(P>0.05);观察组2年生存率显著高于对照组(P<0.05);观察组鼻咽癌复发率、颈部淋巴结转移率和远处淋巴结转移率均显著低于对照组(P<0.05)。观察组CD3+、CD4+/CD8+和CD19+水平均显著高于对照组(P<0.05)。结论替吉奥联合同步推量调强放疗对老年局部晚期鼻咽癌的疗效较好,对免疫功能的负性影响显著低于常规调强放疗,且不良反应发生较低,具有一定的临床应用价值。Objective To investigate the effect of tegafur combined with simultaneous integrated boost intensity modulated radiotherapy(SIB-IMR)on the curation and immune function of the elderly with local advanced nasopharyngeal carcinoma,and so as to find a method for nasopharyngeal carcinoma treatment.Methods 102 elderly cases with local advanced nasopharyngeal carcinoma during the period fromApril,2015 to April,2017 were randomly divided into 2 groups:observation group(n=54)and control group(n=48);cases in observation group were given tegafur combined with SIB-IMR,while cases in control group were given conventional intensity modulated radiotherapy(IMR);The clinical effect and adverse reaction rate and immune function of cases were evaluated and compared between cases of 2 groups.Results.Conclusions Tegafur combined with SIB-IMR is of less adverse reactions,better curative effect and minor negative immune function impact than routine IMR on elderly cases with local advanced nasopharyngeal carcinoma,which has certain clinical value.
