子宫内膜癌中单核细胞趋化蛋白-1表达及其与巨噬细胞浸润的关系

摘　　要：目的:分析子宫内膜癌中单核细胞趋化蛋白-1(MCP-1)表达及其与巨噬细胞浸润的关系。方法:纳入2016年1月-2017年12月60例在本院治疗的子宫内膜癌患者,同时纳入同期在本院治疗的子宫内膜不典型增生患者60例和增生期子宫内膜患者60例。采用免疫组化SP法检测所有患者子宫内膜中的MCP-1蛋白表达和CD68标识巨噬细胞浸润表达情况,分析子宫内膜癌中MCP-1蛋白表达与CD68标识巨噬细胞浸润两者之间的关系。结果:在增生期子宫内膜、子宫内膜癌、子宫内膜不典型增生中MCP-1的阳性表达率分别为11.67%(7/60)、78.33%(47/60)和13.33%(8/60),三者比较差异有统计学意义(P<0.05)。在增生期子宫内膜、子宫内膜癌、子宫内膜不典型增生中CD68标识巨噬细胞的阳性表达率分别为6.67%(4/60)、81.67%(49/60)和18.33%(11/60),三者对比差异有统计学意义(P<0.05)。子宫内膜癌组织中MCP-1蛋白表达与CD68标识巨噬细胞浸润均明显高于增生期子宫内膜和子宫内膜不典型增生患者(P<0.05)。子宫内膜癌组织中MCP-1蛋白与CD68标识巨噬细胞浸润之间呈正相关关系(r=0.913,P<0.05)。结论:子宫内膜癌的发生、发展与MCP-1及巨噬细胞的异常表达之间有密切的关系,子宫内膜癌组织中浸润的MCP-1和巨噬细胞能够促进肿瘤淋巴管和血管的形成,加速肿瘤的浸润和转移。Objective:To analyze the expression of monocyte chemoattractant protein-1(MCP-1)and its relationship with macrophage infiltration in endometrial cancer.Method:Sixty patients with endometrial cancer treated in our hospital from January 2016 to December 2017 were enrolled in the study,meanwhile,60 patients with atypical endometrial hyperplasia and 60 patients with proliferative endometrium were enrolled in the study.Immunohistochemical SP method was used to detect the expression of MCP-1 protein and macrophage infiltration with CD68 marker in endometrium of all patients.The correlation between the expression of MCP-1 protein and macrophage infiltration with CD68 marker was analyzed.Result:The positive expression rates of MCP-1 in proliferative endometrium,endometrial cancer and atypical endometrial hyperplasia were 11.67%(7/60),78.33%(47/60)and 13.33%(8/60),respectively,the difference was statistically significant(P<0.05).In proliferative endometrium,endometrial cancer and atypical endometrial hyperplasia,the positive expression rates of macrophage infitration with CD68 marker were 6.67%(4/60),81.67%(49/60)and 18.33%(11/60),respectively,the difference was statistically significant(P<0.05).The expression of MCP-1 protein and macrophage infitration with CD68 marker in endometrial carcinoma were significantly higher than those in proliferative endometrium and atypical endometrial hyperplasia(P<0.05).There was a positive correlation between MCP-1 protein and macrophage infiltration with CD68 marker in endometrial cancer(r=0.913,P<0.05).Conclusion:The occurrence and development of endometrial cancer are closely related to the abnormal expression of MCP-1 and macrophages,MCP-1 and macrophages infiltrated in endometrial cancer tissue can promote the formation of lymphatic vessels and blood vessels,and accelerate the invasion and metastasis of tumors.
