舒肝解郁胶囊联合利培酮对精神分裂症患者认知中记忆维度的影响

摘　　要：目的探讨利培酮联合舒肝解郁胶囊对精神分裂症患者认知功能中记忆维度的影响。方法采用随机双盲对照方法,选取18~40岁符合ICD-10精神分裂症诊断标准的门诊患者121例,其中61例予利培酮联合舒肝解郁胶囊(研究组),60例予利培酮联合安慰剂(对照组)。在2组基线和治疗24周末,采用霍普金斯词语学习测验-修订版(HVLT-R)、简易视觉空间记忆测验-修订版(BVMT-R)及韦克斯勒空间广度测验-Ⅲ(WMS-Ⅲ)等对2组患者认知功能的记忆维度的差异有统计学意义进行测试和评估;采用Barnes静坐不能量表、锥体外系反应量表及不自主运动量表评估2组药物不良反应。结果治疗24周末与基线时比较:研究组的HVLT-R 1、2试、总分,BVMT-R 1、2、3试及总分,WMS-Ⅲ顺行得分、逆行得分、总分比较差异均有统计学意义(P<0.05);对照组的HVLT-R 1、2、3试、总分及BVMT-R延迟得分比较差异有统计学意义(P<0.05)。治疗24周末与基线差值比较:研究组改善程度均优于对照组,2组HVLT-R 1试、BVMT-R 2试的记忆维度的差异有统计学意义(P<0.05)。2组均未发现明显的药物不良反应。结论舒肝解郁胶囊对精神分裂症患者认知损害中的言语记忆与学习、视觉记忆与学习的改善有一定的辅助作用。Objective To explore the effects of Shuganjieyu capsule combined with Risperidone on memory dimension cognitivefunction in schizophrenic patients.Methods A total of 121 outpatients aged 18-40 years who met the ICD-10 criteria for schizophrenia were selected using the randomized double-blind controlled method.These patients received risperidone combined with either Shuganjieyu capsule(study group,n=61)or placebo(control group,n=60).The cognitive function of memory dimension was assessed using Hopkins verbal learning test-revised(HVLT-R),brief visuospatial memory test-revised(BVMT-R)and Wechsler memory scale-Ⅲ(WMS-Ⅲ)before and after 24 weeks of treatment.The adverse drug reactions were assessed using Barnes akathisia rating scale,a rating scale for extrapyramidal side effects and abnormal involuntary movement scale.Results In experimental group,HVLT-R 1,2 and total scores,BVMT-R 1,2,3 and total scores,and WMS-Ⅲanterograde,retrograde and total scores at the end of 24-week treatment were significantly different from those at baseline(P<0.05).In control group,HVLT-R 1,2,3 and total scores and BVMT-R delayed recall scores at the end of 24-week treatment were obviously different from those at baseline(P<0.05).There were significant,the differences in HVLT-R 1 test and BVMT-R 2 test scores between 24-week treatment and baseline(P<0.05).No adverse drug reactions were found in both groups.Conclusion Shuganjieyu capsule can improve verbal memory and learning and visual memory and learning in cognitive impairment.
