摘　　要：目的探讨腮腺嗜酸细胞癌的CT表现,以提高对该少见疾病的认识和诊断水平。方法回顾性分析经病理证实的15例腮腺嗜酸细胞癌患者的CT表现。对肿瘤的位置、大小、形态、边缘、密度、强化程度、周围侵犯、淋巴结转移等进行分析。结果15例患者均为单发肿瘤,5例位于腮腺浅叶,8例位于腮腺深叶,2例跨深、浅叶;肿瘤最大径0.8~3.6cm,平均(1.7±0.3)cm;病灶呈圆形或椭圆形4例,不规则分叶状11例;边缘光整6例,边缘毛糙9例;病灶与周围组织分界清晰5例,分界不清、侵犯周围结构10例;CT平扫病灶呈等密度10例,低密度5例,所有病灶均未见明显钙化;密度均匀11例,不均匀4例;增强后病灶呈富血供12例,乏血供3例。7例颈部淋巴结转移。结论OCPG的影像表现具有一定的特征性,CT检查可以准确显示肿瘤侵犯程度和淋巴结转移情况,为临床制订治疗方案提供重要依据。Objective To explore the CT imaging findings of oncocytic carcinoma in parotid gland in order to enhance the diagnostic level of this disease.Methods The CT imaging characteristics of 15 patients with pathologically proved OCPG were retrospectively reviewed.CT images were evaluated in relation to the following:location,size,morphology,boundary,attenuation,enhancement pattern,invasion of surrounding structures,and lymph node metastasis.Results Fifteen cases of OCPG had single tumor,five lesions were located in the superficial lobe,8 in the deep lobe and 2 across the superficial and deep lobe.The maximum diameter ranged from 0.8~3.6 cm,with mean of(1.7±0.3)cm.Four cases demonstrated round or oval round masses,eleven cases demonstrated irregular and lobular masses.The boundary was well-defined in 6 cases and ill-defined in 9 cases.Five lesions showed clear margin,ten lesions demonstrated foggy edge and invaded the surrounding structure.10 lesions manifested as iso-density and 5 lesions manifested as low-density on non-enhanced CT,none of fifteen lesions showed calcification.11 lesions displayed uniform density and 4 lesions displayed uneven density.On post contrast images,lesions demonstrated rich blood supply in 12 cases and lack of blood supply in 3 cases.Cervical lymph node metastases were found in 7 cases.Conclusion The imaging findings of OCPG have some characteristics,CT examinations can accurately demonstrate the extent of tumor involvement,lymph node metastasis and are helpful to provide more comprehensive information for clinical management.
