韧带样纤维瘤病的影像表现及其病理特征

摘　　要：目的探讨韧带样纤维瘤的临床、CT及MRI表现并与病理结果进行对照分析,以提高韧带样纤维瘤诊断的准确率。方法回顾性分析经手术及病理证实的12例韧带样纤维瘤患者的临床及影像学资料,并与术后病理进行对照分析。结果12例患者共14枚病灶,其中腹壁型8枚,腹外型5枚,腹内型1枚;12枚呈浸润性生长,2枚呈膨胀性生长,CT平扫肿块呈等稍低密度表现,MRI扫描病灶多呈长T 1混杂T 2信号,内见条索状的低信号带,呈“条带征”,与周围组织边界可见“火焰症”;动态增强3枚呈低-中等程度渐进性强化,3例呈不均匀明显强化。结论韧带样纤维瘤的影像学表现具有一定特征性,结合患者的临床资料可提高本病的诊断准确率。Objective To investigate the clinical perfomance,CT and MRI manifestations,and histopathology features of desmoid fibromatosis(DF),to improve the diagnostic accuracy of this tumor.Methods 12 cases of DF proved by histopathology were retrospectively reviewed,clinical and imaging performance were compared with histopathology results.Results There were 14 masses in 12 cases.8 located in the abdominal wall,5 were extra-abdominal and 1 was intra-abdominal.12 of them were infiltrative growth,and 2 were expansive growth;DF showed hypodense or isodense on plain CT images,this tumor displayed intermediate signal intensity on T 1WI and heterogeneous signal intensity on T 2WI,band sign and flame sign could be seen;3 cases showed low to moderate gradual enhanced on dynamic contrast enhanced CT,3 cases showed heterogeneous markedly enhancement.Conclusion DF has some typical imaging characteristics,which can be used to improve the diagnosis of this tumor when combined with clinical features.
