MR多参数成像对卵巢成熟性囊性畸胎瘤影像征象分析

摘　　要：目的探讨MR多参数成像诊断卵巢成熟性囊性畸胎瘤价值。方法手术病理证实的61例卵巢成熟性囊性畸胎瘤MR多参数成像资料,对照病理,并结合文献综合分析。结果61例共70个病灶,分多脂肪型58个,少脂肪型9个,无脂肪型3个;脂肪型表现为短T1长T2信号,牙齿和钙化在T1WI、脂肪抑制T 2WI表现为低信号;少脂肪型,表现为极少量脂肪信号位于囊壁边缘或Rokitansky结节内。无脂肪型3个,表现为长T1长T2液体信号影。Rokitansky结节、浮球在T1WI、脂肪抑制T2WI为高低混杂信号,脂肪抑制像脂肪信号减低;Rokitansky结节和浮球DWI均高低混杂信号;动态增强扫描,Rokitansky结节轻-中度强化,时间-信号强度曲线(TIC)为I型,浮球无强化。Rokitansky结节3D减影为稍高信号,浮球3D减影无显示。结论MR多参数成像对卵巢成熟性囊性畸胎瘤影像征象提供更多诊断信息,有助于提高对该病的认识和影像诊断正确率。Objective To analysis of Mature Cystic Teratoma of the Ovary by multiparameter MRI.Methods 61 cases of patients with mature cystic teratoma of the ovary were was performed and combined with literature review.Results 61 cases presented with 70 lesions.58 lesions were typical lard type.9 lesions were less fat type.3 lesions were leanness type.The fat type showed typical short T 1 and long T 2 signals.All the teeth and calcification showed low signals.The less fat type showed a less fat is located on the edge of cystic or within the rokitansky.The leanness type showed long T 1 ang long T 2signals.Rokitansky protuberance and the floating ball showed high-low mixed signals and the signal in the fat-suppression sequence decreased.In multi b value of diffusion-weighted imaging,Rokitansky protuberance and floating ball showed high and low signal.The dynamic contrast-enhanced imaging showed little-mild enhancement in the Rokitansky protuberance.The time intensity curve(TIC)of Rokitansky protuberance was type I.The floating ball was without enhancement.Three dimesional subtraction images showed slightly high signal in Rokitansky protuberance the floating ball no display.Conclusion Multimodal MRI is valuable in providing more diagnostic information in mature cystic teratoma of the ovary and helpful to improve the accuracy of diagnosis.
