封闭式负压治疗对改善美容缝合伤口愈合质量的临床研究

摘　　要：目的:观察早期封闭式负压治疗对提高美容缝合伤口愈合质量及降低瘢痕增生程度的效果。方法:60例需要行躯干肿物切除手术的患者随机分成两组,治疗组在伤口行美容缝合后即刻使用封闭式负压吸引治疗,对照组常规换药治疗。对两组患者术后切口愈合质量进行评估,使用患者瘢痕评估量表(PSAS)、温哥华瘢痕量表(VSS)以及视觉模拟量表(VAS),对手术切口瘢痕进行为期1年的随访并行评估打分。结果:在1年的随访中,56例患者被复查,4例患者失访,其中实验组1例,对照组3例。伤口总体愈合质量方面,对照组27例患者中,5例伤口术后出现轻度红肿及皮下少量血肿;治疗组未出现局部红肿及渗出等不良反应。在伤口愈合质量方面两组比较有统计学差异(P<0.05)。PSAS、VSS及VAS评分与对照组相比结果亦均具有统计学差异(P<0.05)。结论:由于可以降低伤口局部张力,减少局部渗出和感染概率。局部封闭式负压吸引治疗可以改善美容缝合伤口的愈合质量,从而起到降低术后伤口瘢痕增生的作用。Objective:To observe the efficacy of early negative pressure treatment on improving the healing quality of cosmetic suture wounds and reducing the degree of scar hyperplasia.Methods:60 patients requiring trunk tumor resection were randomly divided into two groups.The experimental group was treated with closed negative pressure treatment immediately after cosmetic suture,while the control group was treated with routine dressing change.Patients were followed up for 1 year and the healing quality of postoperative incision was assessed.Surgical incision scars were scored by patient scar assessment scale(PSAS),vancouver scar scale(VSS),and visual analog scale(VAS).Results:During the 1-year follow-up,56 patients were reviewed and 4 patients were lost,1 case in the experimental group and 3 cases in the control group.The wound exudation rate and infection rate in the experimental group were lower than those in the control group(P<0.05).After operation,the wounds of 5 cases were mild redness and had a small amount of hematoma in the control group,while there were no adverse reactions such as local redness and exudation in the experimental group.There was statistically significant difference between the two groups in the quality of wound healing(P<0.05).The results of VAS,VSS and PSAS scores were also statistically different from the control group(P<0.05).Conclusion:Closed negative pressure treatment can reduce the local tension,exudation and infection probability of the wound,improve the cosmetic effect of the suture wound,reduce the scar hyperplasia of the wound.
