摘　　要：目的:探讨靛胭脂-醋酸染色对胃黏膜病变的诊断价值。方法:选取内镜下胃黏膜表现异常的118例患者行内镜检查,随机分为白光内镜组57例和染色内镜组61例,于病变处取活检并送组织病理学检查。结果:染色内镜组患者中低级别上皮内瘤变检出率24.59%(15例)及肠上皮化生检出率36.07%(22例)高于白光内镜组患者的10.53%(6例)及17.54%(10例),差异均有统计学意义(P<0.05)。染色内镜组胃黏膜不着色患者中其早期胃癌、高级别上皮内瘤变诊断的灵敏度为84.62%,胃黏膜着色不良患者中其低别内瘤变、肠上皮化生的诊断灵敏度为91.18%;不同的胃黏膜着色结果提示不同的病理结果,差异有统计学意义(P<0.05)。结论:靛胭脂-醋酸染色可明显提高胃早癌及癌前病变诊断率。Objective To investigate the diagnostic value of indigo carmine acetic acid staining in gastric mucosal lesions.Method 118 patients with abnormal gastric mucosa under endoscopy underwent endoscopy.They were randomly divided into white light endoscopy group 51 cases and staining endoscopy group 61 cases.Biopsy was taken at the lesion site and sent to histopathological examination.Results The detection rate of middle and low grade intraepithelial neoplasia(24.5%)and intestinal metaplasia(36.07%)in the dye Endoscopy Group was higher than that in the white light endoscopy group(10.53%)and 17.54%(6 cases),the difference was statistically significant(P<0.05).The sensitivity of early gasthc cancer and high-grade intraepithelial neoplasig was 84.62%and 91.18%respectively in patients with non staining of gastric mucosa in the staining endoscopy group,and the different staining results of gastric mucosa suggested different pathological results with statistical significance(P<0.05).Conclusion Indigo carmine acetic acid staining can significantly improve the diagnostic rate of early gastric cancer and precancerous lesions.
