摘  要:目的:对所收集的缺血性脑卒中病人进行MRI分析与研究,评估不同成像序列在缺血性脑卒中超急性期诊断中的优势,寻找实用可行的MRI序列。方法:收集符合缺血性脑卒中超急性期研究条件的共58例,这58例患者均行常规MRI、DWI、SWI及MRA扫描,对MRA所显示的颅内动脉狭窄、闭塞的阳性率进行记录,对HVS与SVS责任血管的相关性进行分析,以此分析二者对于责任动脉血栓的显示的一致性。结果:58例患者中,FLAIR像上出现HVS者有36例(62.9%),SWI像上出现SVS者有47例(82.3%),MRA显示动脉狭窄或闭塞的患者有51例(89.1%),血栓阳性率SVS>HVS,MRA显示血管狭窄或闭塞的部位与SVS低信号存在的部位一致,经过χ^2检验,证实三者之间的阳性率并不全相等。结论:MRA对显示颅内动脉狭窄、闭塞具有很高的阳性率,SVS与HVS对于责任动脉血栓的存在、位置方面具有很高的一致性,SVS、HVS及MRA对责任动脉的显示基本一致,三者相互补充提高诊断的准确性。Objective To conduct MRI analysis and study on the collected ischemic stroke patients,to evaluate the advantages of different imaging sequences in the diagnosis of ischemic stroke in hyperacute phase,and to find practical and feasible MRI sequences.Methods A total of 58 patients who met the ultra-acute phase of ischemic stroke were enrolled.All 58 patients underwent routine MRI,DWI,SWI,and MRA scans.The positive rate of intracranial artery stenosis and occlusion was recorded by MRA.The correlation between HVS and SVS responsible vessels was analyzed to analyze the consistency of their display of responsible arterial thrombosis.Results Of the 58 patients,36(62.9%)had HVS on the FLAIR,47(82.3%)had SVS on the SWI,and 51(89.1%)had MRA with arterial stenosis or occlusion.The positive rate of SVS>HVS,MRA showed that the location of stenosis or occlusion was consistent with the location of SVS low signal.Afterχ^2 test,the positive rate between the three was not equal.Conclusion MRA has a high positive rate for intracranial stenosis and occlusion.SVS and HVS have high consistency in the presence and location of responsible arterial thrombosis.SVS,HVS and MRA are basically consistent with the display of responsible arteries.Complement each other to improve the accuracy of the diagnosis.
