摘 要:目的:探讨高速涡轮牙钻联合微创拔牙刀对下颌近中阻生智齿患者疗效及龈沟液前列腺素E2(PGE2)、降钙素基因相关肽(CGRP)水平的影响。方法:选取2017年1月至2018年6月广东省东莞市人民医院收治的下颌近中阻生智齿患者86例,按随机数字表法分为试验组(n=43)和对照组(n=43)。对照组以传统拔牙方法施行拔牙,试验组以高速涡轮牙钻联合微创拔牙刀施行拔牙。比较两组手术情况、术中畏惧程度、术后并发症发生率、疼痛程度及术前、术后1 d、3 d龈沟液PGE2、CGRP水平。结果:试验组手术时间较对照组短,术后肿胀度、张口受限度及拔牙完整性评分较对照组低(P<0. 05);试验组术中畏惧程度、术后并发症发生率、疼痛程度较对照组低(P<0. 05);两组术后1 d、3 d龈沟液PGE2、CGRP水平均较术前提高,但试验组低于对照组(P<0. 05)。结论:下颌近中阻生智齿患者予以高速涡轮牙钻联合微创拔牙刀手术治疗,操作更为便捷,且可减轻术中畏惧感,降低术后肿胀、疼痛及张口受限程度,提升拔牙窝完整性,减少并发症发生。Objective: To investigate the effect of high-speed turbine dental drill combined with minimally invasive extraction knife on postoperative mouth opening restriction and gingival crevicular fluid prostaglandin E2(PGE2)and calcitonin gene-related peptide(CGRP)levels in patients with mandibular proximal and middle impacted wisdom teeth. Methods: Eighty-six patients with mandibular near-middle impacted wisdom teeth in our hospital from January 2017 to June 2018 were enrolled,and randomly divided into an experimental group(n=43)and a control group(n=43). The patients in the control group received traditional teeth extraction,and those in the experimental group received teeth extraction with high-speed turbine drill combined with a minimally invasive extraction knife. The operation time,postoperative swelling,mouth opening limit and tooth injury degree,intraoperative fear,postoperative complication incidence,and pain degree as well as the PGE2 and CGRP levels in gingival crevicular fluid were compared between the two groups. Results: The operation time,postoperative swelling degree,mouth opening restriction degree,tooth injury degree,fear degree,postoperative complications incidence,pain degree and the levels of PGE2 and CGRP at 1 and 3 days post-operation were lower than those in the control group(P<0. 05). Conclusion: High-speed turbine drill combined with minimally invasive extraction knife for mandibular proximal impacted wisdom teeth is more convenient,and it can reduce teeth injury,intraoperative fear,postoperative complications,swelling,pain and mouth limitation.
