基于临证处方探析五运六气与经方方证相应的关系

摘　　要：目的:从方证相应处方探析运气学规律与经方方证相应的关系。方法:采集了2015年9月-2018年4月,以方证相应方法研究经方的代表人物之一黄煌教授的临床方证相应处方2 124张,结合五运六气理论,用统计学方法进行处方规律分析。结果:(1)总计采集处方2 124张,涵盖88种方剂;其中,类方总数1 801张,涵盖本研究的全部12种类方,类方处方数占总处方数的84.8%。(2)柴胡类方、桂枝类方、半夏类方、黄连类方、麻黄类方、栀子类方、黄芩类方、附子类方这8类类方临床开立最多,总计1 743张,占处方总数的96.8%,尤其是柴胡类方,占比超过了三分之一;差异具有统计学意义(P<0.001)。(3)2016丙申年、2017丁酉年柴胡类方、黄连类方、附子类方两年数据接近;2017丁酉年桂枝类方、黄芩类方的使用明显增多;而半夏、麻黄、栀子类方的使用明显减少,差异具有统计学意义。结论:将五运六气理论阐述的疾病规律作为临床方证相应理论遣方用药的证据之一,拓展方证相应理论的内涵和外延是可行的。Objective:To explore the relationship between the Five Circuits and Six Qi and Clinical Prescription based on the theroy of correspondence between prescription and syndrome.Methods:During the period from September 2015 to April 2018,Huanghuang’s prescriptions were collected based on the theory of correspondence between prescription and syndrome.And the dataes were analyzed based on the theory of Five Circuits and Six Qi.Results:(1) A total of 2 124 prescriptions were collected,covering 88 classes prescriptions;among them,the total number of prescriptions belong the 88 classes is 1 801,covering all 12 classes studied in this paper.The number of class prescriptions accounts for 84.8% of the total number of prescriptions.(2)The prescription’s classes of Radix Bupleuri,Ramulus Cinnamomi,Elaeagnus courtoisi,Coptis chinensis Franch.,Ephedra sinica Stapf.,Gardenia Ellis,Scutellaria Linn.were the most,totaling 1 743,accounting for 96.8% of the total number of prescriptions.In particular,the class of Radix Bupleuri,accounted for more than one-third;The difference is statistically significant(P<0.001).(3) In 2016,the data for the two years of 2017,the numberes of prescriptions of classes of Radix Bupleuri,Coptis chinensis Franch,Aconitum carmichaeli Debx were close;In 2017,the use of prescription’s classes of Ramulus Cinnamomi,and Scutellaria Linn were significantly increased;The classes of Elaeagnus courtoisi,Ephedra sinica Stapf,Gardenia jasminoides Ellis were significantly reduced,and the difference was statistically significant.Conclusion:There is a relationship between the Five Circuits and Six Qi and the theroy of Correspondence between Prescription and Syndrome.
