摘 要:目的比较三维适形放射治疗(3DCRT)、调强技术(IMRT)在食管癌的剂量学差异。方法选择20例食管癌患者,为每例患者制定3组放疗计划:3DCRT、4野、7野计划。处方剂量为PGTV:60 Gy,PTV:50 Gy,分为26~27次照射。比较不同靶区和危及器官剂量学参数。结果4野IMRT计划CIGTV、HIPTV、CIPTV三个指数优于3D-CRT计划,4野IMRT计划CIPTV指数优于7野计划,各剂量指标的差异具有统计学意义(P<0.05);肺的受照剂量中,4野IMRT计划中V20参数优于3DCRT计划,V20、V30、V5参数优于7野IMRT计划,其差异具有统计学意义(P<0.05);三组计划中脊髓Dmax<45 Gy,心脏V33<46%,各组参数无统计学意义差别(P>0.05)。结论3DCRT不能很好的提高食管癌靶区剂量和保护重要器官的剂量,4野IMRT与7野IMRT相比,心脏、脊髓剂量差异不显著,但是肺的受量明显降低且患者接受照射的时间较7野IMRT减少2倍,治疗费用较7野调强减少1倍。Objective To compare three dimensional conformal radiotherapy(3 DCRT),IMRT in the dosimetric differences of esophageal carcinoma.Methods Choose 20 patients with esophageal cancer.The formulation of 3 set radiotherapy plans for each patients:3 DCRT,4 fields,7 fields IMRT plan.The prescription dose was PGTV:60 Gy and PTV:50 Gy,was divided into 27~26 times.Compare the dosimetric parameters of different plan targets and endanger organs.Results 4 fields IMRT plan CIGTV,HIPTV,CIPTVthree index was better than the 3 D-CRT plan,The CIPTV index of 4 fields IMRT plan was better than 7 fields plan,and the difference of each dose was statistically significant(P<0.05);According to the dose of the lung,the V20 parameters in the 4 fields IMRT plan was better than the 3 DCRT plan,V20 and V30 parameters were better than the 7 fields IMRT plan,and the V5 parameter was better than the 7 field IMRT plan,the difference was statistically significant(P<0.05).The Dmax<45 Gy of the spinal cord and the cardiac V33<46%in the 3 groups were not statistically significant(P>0.05).Conclusion 3 DCRT could not improve the dose of the target area and protect the vital organs of the esophageal cancer.4 fields IMRT and 7 fields IMRT compared to the heart,spinal cord dose difference is not significant,But the amount of lung is significantly reduced and the patients received irradiation time than 7 fields IMRT reduced by 2 times,The costs compared to 7 field strength reduce 1 times.
