摘　　要：目的探讨2型糖尿病(T2DM)患者血清糖类抗原19-9(CA19-9)水平与糖尿病肾病(DN)的关系。方法选取2016年1月至2018年12月我院内分泌科收治的T2DM患者278例,同时选取年龄、性别匹配的正常对照(NC)组90例。根据尿蛋白/肌酐比值(UACR)将T2DM患者分为3组,即正常白蛋白尿(DM)组96例、微量白蛋白尿(DN1)组94例、大量白蛋白尿(DN2)组88例。比较各组血清CA19-9水平;采用Pearson相关分析和Logistic回归分析探讨UACR与CA19-9的关系。结果血清CA19-9在NC、DM、DN1、DN2组水平依次升高,组间及组内两两比较差异均有统计学意义(P<0.05)。T2DM患者UACR与CA19-9呈显著正相关(r=0.36,P<0.01)。Logistic回归分析显示UACR是T2DM患者CA19-9水平升高的独立危险因素。结论T2DM患者血清CA19-9水平升高,且随着尿蛋白排泄的增多而进一步升高,临床判读时应结合其UACR水平综合考虑,以免误诊。Objective To investigate the association between CA19-9 and diabetic nephropathy(DN)in patients with type 2 diabetes mellitus(T2DM).Methods From January,2016 to December,2018,278 T2DM patients and 90 normal control subjects(NC group)were enrolled for the study.Patients were divided into 3 groups according to urinary albumin-to-creatinine ratio(UACR):normoalbuminuria group(DM,n=96),microalbuminuria group(DN1,n=94)and macroalbuminuria group(DN2,n=88).The levels of CA19-9 were compared among the four groups.Pearson correlation analysis and logistic regression analysis were used to analyze the association between CA19-9 and UACR.Results CA19-9 levels increased progressively from NC,DM,DN1,then to DN2 group,and there were significant differences among groups(P<0.05).CA19-9 was positively and significantly associated with UACR in T2DM patients(r=0.36,P<0.01).Logistic regression analysis showed that UACR was an independent risk factor for elevated serum CA19-9 levels in T2DM patients.Conclusion CA19-9 levels are significantly higher in T2DM patients,and CA19-9 levels show a gradual and markedly increase with increasing of urinary albumin excretion.UACR should be taken into consideration for clinical interpretation to avoid misdiagnoses.
