摘 要:目的本文探讨了新疆地区汉族老年人阿尔茨海默病(Alzheimer disease,AD)和轻度认知功能障碍(mild cognitive impairment,MCI)患者外周血DNA绝对端粒长度(absolute telomere length,ATL)的差异,为进一步研究AD及MCI发病机制提供理论基础。方法选择在2017年1月至2018年5月于新疆医科大学第一附属医院老年病科就诊的年龄≥60岁的患者,根据诊断及排除标准入选AD组37例(男20例,女17例),MCI组63例(男27例,女36例),对照组126例(男51例,女75例),采集所有受试者晨起空腹血,通过实时荧光法(RTPCR)检测所有样本的ATL,比较其在三组人群中的差异。结果 MCI组ATL 0.0464(0.0456,0.0476)短于对照组0.0479(0.0463,0.0501)(P=0.0006),且短于AD组0.0503(0.0446,0.0543)(P=0.001)。在男性、患高血压及年龄在76~90岁者中MCI组ATL短于对照组及AD组(P<0.05);在女性、未患高血压、高中以下学历及年龄在60~75岁者中MCI组ATL短于对照组(P<0.05);在高中及以上学历者中MCI组ATL短于AD组(P=0.015)。结论外周血DNA的ATL可能影响新疆汉族人群AD及MCI的发病,且在AD患者中可能存在某种维持端粒长度稳定的机制。Objective The study examined absolute telomere length(ATL) in peripheral blood DNA of AD and MCI patients in Han nationality in Xinjiang to provide a theoretical basis for further study of pathogenesis of AD and MCI.Methods The patients aged 60 years and over who were admitted to the Department of Geriatrics of the First Affiliated Hospital of Xinjiang Medical University from January 2017 to May 2018 were enrolled according to the diagnosis and exclusion criteria.There were 37 patients in AD group(20 males and 17 females), 63 patients in MCI group(27 males and 36 females), and 126 normal people as control group(51 males and 75 females).Real-time fluorescence(RT-PCR) was used to test ATL for all samples and their ATL differences were compared among the three groups as well as subgroups.Results The ATL of MCI group was significantly shorter than control group(P =0.0006),and AD group(P =0.001).In males, ATL in MCI group was shorter than control group and AD group in patients with hypertension and those aged 76 ~90 years(P <0.05).In females, ATL in MCI group was shorter than control group in patients without hypertension, with high school education or aged 60 to 75 years old(P <0.05).ATL in MCI group was shorter than AD group(P =0.015) in patients with high school education and above.Conclusion ATL of peripheral blood DNA may affect the pathogenesis of AD and MCI in Xinjiang Han population via some mechanisms for maintaining telomere length stability in AD patients.
