NBI内镜下NICE分型对结直肠息肉诊断的临床研究

摘　　要：目的 在窄带成像技术(NBI)内镜下分析探讨国际结直肠内镜分型(NICE分型)对结直肠息肉诊断的临床价值.方法 回顾性分析60例结直肠息肉患者,通过观察患者病变部位的表面结构、病变颜色以及微血管结构三方面并结合NICE分型对病变性质进行预测,观察NICE分型诊断及病理检查结果 ,并分析NICE分型在NBI内镜下诊断病变范围不同的结直肠肿瘤的情况.结果120处病变部位中,依据NICE分型诊断,其中Ⅰ型共66处,Ⅱ型共54处;依据病理诊断,非肿瘤性共66处,肿瘤性共54处.随着病变不断增大,NBI内镜下对病变范围不同结直肠肿瘤诊断的准确率也提高,其中病变范围≤5 mm为92.2%,病变范围6~9 mm为97.6%,病变范围≥10 mm为96.3%.结论 在NBI内镜下通过NICE分型标准可以准确区分患者结直肠息肉属于肿瘤或非肿瘤病变,使患者能够得到早一步的针对性治疗,指导意义良好,值得临床推广.Objective To discuss the clinical value of narrow band imaging international colorectal endoscopic(NICE)classification under narrow band imaging(NBI)endoscopy in the diagnosis of colorectal polyps.Methods The clinical data of 60 patients with colorectal polyps were retrospectively analyzed.The nature of the lesion was predicted by observing the surface structure,color and microvascular structure of the lesion site and combining NICE classification.The results of NICE classification diagnosis and pathological examination was observed,and the diagnostic situation of colorectal tumors with different pathological ranges by NICE classification under NBI endoscopy was analyzed.Results In 120 lesions,there were 66 typeⅠlesions and 54 typeⅡlesions according to NICE classification,while 66 non-tumorous lesions,and 54 tumorous lesions according to pathological diagnosis.With the increase of lesions,the accuracy of NBI endoscopy in the diagnosis of colorectal tumors with different lesion ranges has been improved.with lesions range≤5 mm as 92.2%,lesions range 6-9 mm as 97.6%,and lesions range≥10 mm as 96.3%.Conclusion Under NBI endoscopy,patients with colorectal polyps can be accurately classified as tumors or non-tumorous lesions by NICE classification criteria,so that patients can get early targeted treatment.It has good guiding significance and is worthy of clinical promotion.
