快速扩弓和推磨牙远移联合应用对牙列拥挤伴牙弓狭窄的青少年患者的疗效研究

摘　　要：目的分析研究使用快速扩弓结合推磨牙远移对牙列拥挤伴牙弓狭窄青少年患者的疗效,以供临床治疗牙列拥挤提供参考。方法以2016年1月~2017年12月共60例牙列拥挤伴牙弓狭窄患者为研究对象,患者均给予快速扩弓和推磨牙远移联合治疗方法,对比观察患者的治疗效果及牙量、基骨及Howes测定结果,评价PAR分值。结果治疗后,患者牙弓宽度PMD、基骨宽度PMBAW增加,BAL增加,PMD/TM、PMBAW/TM、BAL/TM增加,与治疗前比较,P<0.05,差异有统计学意义。治疗后,60例患者PAR分值(牙排列、覆盖、覆苓、中线、近远中苓关系、横向苓关系、总分值)均有明显降低,与治疗前比较,P<0.05,差异有统计学意义。结论使用快速扩弓结合推磨牙远移方法对牙列拥挤伴牙弓狭窄青少年有较好的效果,矫正效果稳定,值得应用。Objective To analyze the curative effect of the rapid arch expansion on adolescents with crowded dentition combined with stenosis,and to provide a reference for the clinical treatment of crowded dentition.Methods From January 2016 to December 2017,a total of 60 patients with dentition with dental arch stenosis were enrolled in the study.All patients were treated with rapid expansion and distal molar transfer.The therapeutic effect and teeth of the patients were compared.The PAR score was evaluated through the amount,the base bone and the Howes measurement.Results After treatment,the patient's dental arch width PMD,base bone width PMBAW increased,BAL increased,PMD/TM,PMBAW/TM,BAL/TMincreased,compared with before treatment,P<0.05,and the difference was statistically significant.After treatment,PAR scores of 60 patients(including tooth alignment,coverage,ridge,midline,near-distal relationship,lateral sputum relationship and total score)were significantly reduced compared with those before treatment,P<0.05,and the differences were statistically significant.Conclusion The method of rapid expansion combined with the molar grinding method has a good effect on the adolescents with dentition and stenosis,and the correction effect is stable and is worthy of application.
