调中益气汤对碘-125放射性粒子支架置入术后中晚期食管癌患者吞咽功能及血清肿瘤标志物的影响

摘　　要：目的观察调中益气汤对碘-125放射性粒子支架置入术后中晚期食管癌患者吞咽功能及血清肿瘤标志物的影响。方法将76例中晚期食管癌患者按照随机数字表法分为2组。对照组38例采用碘-125放射性粒子支架置入术治疗;治疗组38例在对照组治疗基础上,术后第3 d予调中益气汤口服,连续治疗4周。观察2组患者支架置入情况;观察2组治疗前后Stooler评分、欧洲癌症研究与治疗组织生命质量测定量表(EORTC QLQ-C30)评分及食管癌患者补充量表(QLQ-OES18)评分变化;观察2组治疗前后中医证候评分变化;观察2组治疗前后血清肿瘤标志物鳞状细胞癌相关抗原(SCC-Ag)、癌胚抗原(CEA)及糖类抗原199(CA199)水平变化。结果2组治疗后Stooler评分、QLQ-OES18评分均较本组治疗前降低(P<0.05),EORTC QLQ-C30评分升高(P<0.05);且治疗组Stooler评分、QLQ-OES18评分降低更明显(P<0.05),EORTC QLQ-C30评分升高更明显(P<0.05)。2组治疗后中医证候各项评分及总评分均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。2组治疗后SCC-Ag、CEA及CA199水平均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。结论对碘-125放射性粒子支架置入术后中晚期食管癌患者给予调中益气汤辅助治疗,能明显改善患者吞咽困难情况,降低血清肿瘤标志物水平,有助于缓解病情,提高生命质量,安全性高。Objective To observe the clinical efficacy of Tiaozhong Yiqi decoction on the swallowing function and serum tumor markers in patients with advanced esophageal cancer after iodine-125 radioactive particle stent implantation.Methods 76 patients with advanced esophageal cancer were randomly divided into two groups according to random number table method.38 cases in control group were treated by iodine-125 radioactive particle stent implantation.38 cases in treatment group were treated by Tiaozhong Yiqi decoction on the third day after operation on the basis of the treatment in the control group for 4 weeks.The stent implantation condition in two groups were observed.The Stooler score,european cancer research and treatment organization quality of life scale(EORTC QLQ-C30)score and esophageal cancer patients supplementary scale(QLQ-OES18)score before and after treatment were observed in two groups.The changes of TCM syndrome score before and after treatment were observed in two groups.The levels of serum tumor marker squamous cell carcinoma antigen(SCC-Ag),carcinoembryonic antigen(CEA)and carbohydrate antigen199(CA199)were observed before and after treatment in two groups.Results After treatment,the Stooler score,QLQ-OES18 score of the two groups were lower than that in the group before treatment(P<0.05),while EORTC QLQ-C30 score were higher than that in the group before treatment(P<0.05).The decrease of Stooler score,QLQ-OES18 score in treatment group was more obvious(P<0.05).The increase of EORTC QLQ-C30 score in treatment group was more obvious(P<0.05).After treatment,the each item score and total scores of TCM syndrome in the two groups were lower than those in the group before treatment(P<0.05),and the decrease in treatment group was more obvious(P<0.05).After treatment,SCC-Ag,CEA and CA199 in the two groups were lower than those in the group before treatment(P<0.05),and the decrease in treatment group was more obvious(P<0.05).Conclusion The application of Tiaozhong Yiqi decoction in the treatment of patients wit
