初始力值对远中移动牙龈沟液TIMP-2和PGE2水平的影响

摘　　要：目的:探究不同远中移动初始力对错[牙合]畸形患者金属蛋白酶组织抑制物(Tissue inhibitor of metalloproteinase,TIMP-2)、前列腺素E2(Prostaglandin E2,PGE2)水平的影响。方法:选取2015年4月-2018年3月笔者医院收治的114例错[牙合]畸形患者为研究对象,按照随机数字法分为A、B、C三组,每组38例,所有患者均接受正畸治疗,各组给予患者50g、100g、200g的远中移动初始力,分别对两侧上颌尖牙于加力前、加力后1、2、3、4周检测TIMP-2,1周内检测PGE2水平并记录。结果:治疗1、2、3、4周后,B组龈沟液TIMP-2水平明显高于A、C组(P<0.05);龈沟液PGE2水平在24h、48h明显升高,在168h后逐渐回到基线水平;B组治疗1h、24h、48h、72h龈沟液PGE2水平明显高于A、C组(P<0.05)。结论:错[牙合]畸形患者龈沟液中TIMP-2、PGE2水平与不同远中移动初始力及持续时间相关,100g作用力下是最佳矫治力,同时TIMP-2、PGE2水平可作为远中移动初始力重要参考指标。Objective To investigate the effects of different distal mobileinitial forces on t TIMP-2 and PGE2 levels in patients with malocclusion.Methods A total of 114 patients with malocclusion admitted in our hospital between April 2015 and March 2018 were divided into the group A,group B and group C according to the random number method with 38 cases in each group.All patients received orthodontic treatment and were given initial force of 50g,100g and 200g in the distal movement,repectively.TIMP-2 was detected on both sides of the maxillary canine before stress and 1,2,3 and 4 weeks after the stress.PGE2 levels were measured and recorded within one week.Results The level of TIMP-2 in the gingival crevicular fluid(GCF)of group B was significantly higher than that of group A and group C 1,2,3 and 4 weeks after treatment(P<0.05).The level of PGE2 in GCF was significantly increased at 24h and 48h and gradually decreased to baseline after 168h;PGE2 levels in GCF were significantly higher in the group B than in the group A and group C at 1h,24h,48h and 72h(P<0.05).Conclusion The levels of TIMP-2 and PGE2 in GCF of patients with malocclusion are related to the different distal mobile initial force and duration,the best under 100g force.Meanwhile,the TIMP-2 and PGE2 levels can be used as the important reference indexes fordistal mobile initial force.
