摘  要:目的观察近视患者配戴角膜塑形镜后角膜前表面中央和旁中央(相对)高度的变化规律。方法观察及对比24例近视患者在戴镜前及戴镜后1 d、1~2周及1个月角膜地形图;右眼角膜中心及水平、垂直子午线距角膜中心4、3、2、1 mm共17点(分别记为:颞4、颞3、颞2、颞1;中心;鼻1、鼻2、鼻3、鼻4;上4、上3、上2、上1;下1、下2、下3、下4)的(相对)高度;戴镜前角膜各点(相对)高度的分布特征及戴镜后各时间点各点(相对)高度的变化趋势。结果戴镜前角膜前表面水平及垂直方向的(相对)高度分布不对称。且鼻侧高度较颞侧高,上方高度较下方高,差异均有统计学意义(F=57.774,23.839;P=0.000,0.000)。各点的(相对)高度在戴镜后1 d开始变化,1~2周时变化最大且渐趋稳定。与戴镜前相比,颞侧周边至鼻侧旁中央区均不同程度升高,鼻侧周边高度降低;上方、下方周边高度均下降,其余各点高度均升高。角膜前表面颞3、鼻2,上2、下3点高度升高最为明显。结论角膜鼻侧周边高度较高,余角膜各点均低;鼻侧较颞侧高,上方较下方高。戴角膜塑形镜后:戴镜后角膜前表面形态渐趋于镜面对称。水平子午线方向除鼻侧周边高度下降外,余各点高度均升高;垂直子午线方向除上、下方周边高度下降外,余各点高度也均有升高。戴镜后1~2周镜片中央定位趋于稳定。戴镜后1~2周为判断镜片中央定位是否合适的最早时间点。镜片塑形区域偏颞侧及下方位,且颞侧偏位比下方偏位更为明显。Objective To observe the changing rule in the central and peripheral(relative)height of anterior corneal surface after the wearing of orthokeratology lens.Methods The corneal topography of 24 myopia patients before the lens wearing and at 1d,1-2 week and 1 month after the lens wearing was observed and compared;the(relative)height of a total of 17 points was observed and compared;the distribution characteristics of the(relative)height of each point of corneal before the lens wearing and the changing trend in the(relative)height of each point of corneal at each time point after lens wearing were observed and compared.Results The(relative)height of anterior cornea surface was asymmetrically distributed horizontally and vertically.The nasal side was higher than the temporal side and the upper side was higher than the lower side,and the differences were statistically significant(F=57.774,23.839;P=0.000,0.000).The(relative)height of each point started to change on 1d after lens wearing and had the greatest change at 1-2 weeks and then became stable.Comparing with those before lens wearing,the points at the central area beside the temporo and the nasal side had elevation of varying degree and the height of points at the nasal side was decreased;the height of the points on the upper and lower part was decreased and the height of other points was increased.Conclusion The central positioning of the lens becomes stable at 1-2 weeks after the lens wearing.1-2 weeks after lens wearing is the earliest time point to judge whether the central positioning of lens is appropriate.
