摘 要:目的运用微血管成像(SMI)评估穿支血管对超声乳腺影像报告和数据系统(US BI-RADS)4类微小(≤1cm)肿块的诊断价值。方法85例患者,共87个最大直径≤1cm US BI-RADS 4类肿块,联合穿支血管对其分类进行调整,采用受试者操作特征(ROC)曲线评价US BI-RADS、穿支血管与US BI-RADS联合应用对US BI-RADS 4类微小肿块的诊断价值。结果87个乳腺微小肿块中,良性46个,恶性41个。调整前US BI-RADS分类、联合穿支血管调整后US BI-RADS分类对恶性肿块诊断的灵敏度、特异度和准确率分别为82.9%vs 73.2%、52.2%vs 78.3%、66.7%vs 75.9%。结论联合穿支血管调整后的US BI-RADS分类能够明显提高诊断的特异度和准确率,有助于鉴别乳腺BI-RADS 4类微小肿块的良恶性。Objective To evaluate the diagnostic value of superb micro vascular imaging(SMI)for detecting penetrating vessels in Ultrasound Breast Imaging Reporting and Data System(US BI-RADS)category 4 micro lesions(≤1 cm).Methods 87 lesions(≤1 cm)in 85 patients were classified as US BI-RADS category 4 lesions and adjusted with penetrating vessels.Receiver operating characteristic(ROC)curve was used to evaluate the diagnostic value of US BIRADS and the combined use of the penetrating vessels and US BI-RADS.Results Among 87 breast lesions,46 were benign and 41 were malignant.The sensitivity,specificity and accuracy of US BI-RADS category before and after adjustment for the diagnosis of malignant lesions were 82.9% vs 73.2%,52.2% vs 78.3%,66.7% vs 75.9% respectively.Conclusions After adjustment with penetrating vessels,US BI-RADS category can improve the specificity and accuracy of diagnosis and can be helpful in differentiating benign and malignant breast BI-RADS category 4 micro lesions.
