摘 要:目的总结头颈部IgG4相关性疾病(IgG4-RD)的超声表现特征。方法回顾性分析22例头颈部IgG4-RD超声图像,总结累及不同器官的声像图特征,包括部位、形态、边界、回声、血流情况等。结果22例患者分别见1~4个头颈部器官受累,其中单器官受累12例,2个器官受累7例,3个器官受累2例,4个器官受累1例。涎腺(颌下腺13例,腮腺5例)受累表现为局限型结节性病变和弥漫型病变。泪腺(4例)受累表现为双侧对称性增大,回声减低,可见条索状强回声。甲状腺(2例)受累表现为低回声区,形态不规则,边界不清。淋巴结(11例)受累无特异度,常表现为边界清楚,髓质减少或消失,皮髓质分界清或不清,钙化及液化少见。结论头颈部IgG4-RD声像图表现具有一定特征性,超声可作为头颈部IgG4-RD首选影像学检查方法。Objective To summarize the ultrasonographic features of IgG4-related diseases(IgG4-RD)of the head and neck.Methods The ultrasonographic data of 22 cases of IgG4-RD of the head and neck were analyzed retrospectively.The sonographic features of different organs involved were summarized,including location,shape,boundary,echo and blood flow.Results Among the 22 patients,1-4 head and neck organs were involved,12 cases were involved in 1 organ,7 cases were involved in 2 organs,2 cases were involved in 3 organs and 1 case was involved in 4 organs.The involvement of salivary glands(13 cases of submandibular gland and 5 cases of parotid gland)showed localized nodular lesions and diffuse lesions.The lacrimal gland(4 cases)was characterized by bilateral symmetrical enlargement,low echo and strong stripe like echo.Thyroid(2 cases)showed low echo area,irregular shape and unclear boundary.The involvement of lymph nodes(11 cases)was nonspecific and often manifested as clear margin,reduction or disappearance of medulla,clear or unclear demarcation of cortex and medulla,and calcification and liquefaction were rare.Conclusions The sonographic images of IgG4-RD of the head and neck has certain characteristics,Ultrasound can be used as the first choice of imaging examination for IgG4-RD of the head and neck.
