烤瓷冠基牙折断后利用原修复体再修复的效果

摘　　要：目的了解烤瓷冠基牙折断后利用原修复体再修复的效果。方法选择烤瓷冠或桥的基牙折断病人52例(68颗),随机数字表法分为原修复体修复组27例(35颗)和重新制作修复组25例(33颗)。原修复体修复组基牙折断的烤瓷冠或桥通过直接形成纤维桩核使得原修复体在原折断的基牙继续使用;重新制作修复组重新制作桩核,重新预备后重新制作修复体。随访2年,观察并比较两组临床效果。采用Kaplan-Meier法绘制生存曲线,log-rank检验分析比较2组患牙修复后2年的成功率。结果原修复体修复组35颗患牙成功率为88.57%,与重新制作修复组成功率比较差异无统计学意义(P>0.05),两组并发症发生率比较差异无显著性(P>0.05)。结论使用纤维桩利用原修复体进行烤瓷冠基牙折断后的再修复是可行、有效的,值得临床推广。Objective To investigate the effect of re-repair with original restorations after fracture of abutment teeth of porcelain-fused-to-metal crown.Methods A total of 52 patients(68 teeth)with fracture of abutment teeth of porcelain-fused-to-metal crowns or bridges were selected,and they were divided into 27 cases(35 teeth)in original restoration group and 25 cases(33 teeth)in reconstructed restoration group according to the random number table method.The porcelain-fused-to-metal crowns or bridges with broken abutments in the original restoration group continued to use the original restorations in the original broken abutments through the direct formation of fiber post-core.In the reconstructed restoration group,the post and core were remade,and the prosthesis was remade after re-preparation.The patients were followed up for 2 years,and the clinical effects were observed and compared between the two groups.The Kaplan-Meier method was used to plot survival curves,and log-rank test was used to analyze and compare the success rate at 2 years after restoration between the two groups.Results The success rate of 35 teeth in the original restoration group was 88.57%,which had no significant difference compared with the reconstructed restoration group(P>0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion Re-repair with original restorations after fracture of abutment teeth of porcelain-fused-to-metal crown with fiber posts is feasible and effective,which is worthy of clinical application.
