人体用组织胶水注射治疗急性非静脉曲张性上消化道出血的临床疗效观察

摘　　要：目的观察人体用组织胶水栓塞治疗急性非静脉曲张性上消化道出血(ANVUGIB)的疗效。方法将80例ANVUGIB患者随机分为观察组和对照组,每组40例。在常规治疗的基础上,观察组患者在胃镜下用人体用组织胶水行裸露血管、出血病灶内注射治疗,对照组胃镜下用金属钛夹止血或喷洒重酒石酸去甲肾上腺素止血治疗。观察并比较两组患者一次止血成功率、1周内再次出血率、止血操作时间、住院总费用及住院时间。结果观察组一次止血成功率为100.0%,高于对照组的90.0%,1周内再次出血率为5.0%,低于对照组的22.2%(均P<0.05);观察组患者止血操作时间、住院时间均短于对照组,住院总费用高于对照组(均P<0.05)。结论人体用组织胶水病灶栓塞治疗ANVUGIB止血成功率高,再次出血率低,住院时间短,但费用相对较高。Objective To observe the efficacy of embolization with human tissue glue for acute non-variceal upper gastrointestinal bleeding(ANVUGIB).Methods Eighty patients with ANVUGIB were randomly divided into observation group and control group,with 40 cases in each group.On the basis of routine treatment,injection into exposed blood vessels and bleeding lesion using human tissue glue under gastroscope was conducted in patients of the treatment group,while the control group received hemostatic therapy with metal titanium clip or noradrenaline bitartrate under gastroscope.The success rate of one-time hemostasis,the rate of re-bleeding within one week,the operation time of hemostasis,the total hospitalization cost and hospital stay in the two groups were observed.Results The success rate of one-time hemostasis in the treatment group was 100.0% and higher than 90.0% in the control group;the rate of re-bleeding within one week in the treatment group was 5.0% and lower than 22.2% in the control group(all P<0.05).Compared to the control group,patients in the treatment group had shorter operation time of hemostasis and hospital stay,but higher total hospitalization cost(all P<0.05).Conclusion Embolization with human tissue glue for ANVUGIB has a high success rate of hemostasis,a low re-bleeding rate,a short hospital stay,but a relatively high expense.
