磁共振波谱代谢物比值在前列腺癌诊断中的应用价值探讨

摘　　要：目的探讨前列腺癌磁共振波谱代谢物比值(9Cho+3Cr)/4Ci在诊断中的临床价值,寻找到(9Cho+3Cr)/4Ci比值对于中央叶与外周带前列腺癌病灶诊断的最佳阈值。方法收集经病理证实的前列腺癌患者的波谱分析结果,分析(9Cho+3Cr)/4Ci比值对中央带及外周带前列腺癌诊断的参考值。结果前列腺癌中央带及外周带代谢物比值有显著差异(P<0.001);中央叶与外周带ROC曲线下面积分别为0.933和0.948。当中央叶(9Cho+3Cr)/4Ci的比值为1.63时,诊断中央叶前列腺癌的灵敏度、特异度为91.7%和92.5%;当外周带(9Cho+3Cr)/4Ci的比值取0.73时,诊断外周带前列腺癌的敏感度、特异度为93.3%和99.1%。结论(9Cho+3Cr)/4Ci比值对前列腺癌的诊断价值较高,并且(9Cho+3Cr)/4Ci在外周带中的价值优于中央带。Objective To investigate the clinical value of the metabolite ratio of magnetic resonance spectrum of prostate cancer(9Cho+3Cr)/4Ci in diagnosis,and to find the optimal threshold for the diagnosis of central leaf and peripheral prostate cancer lesions(9Cho+3Cr)/4Ci ratio.Methods The spectrometry analysis of pathologically confirmed prostate patients was collected,and the ratio of(9Cho+3Cr)/4Ci was analyzed to the measure of central and peripheral prostate cancer.Results There were significant differences in the ratio of metabolites in the central and peripheral band of prostate cancer(P<0.001),and the area under the ROC curve of the central leaf and peripheral band was 0.933 and 0.948,respectively.Using the optimal(9Cho+3Cr)/4 Ci threshold of 1.63 in the central zone,the sensitivity and specificity in detecting cancer were 91.7%and 92.5%,respectively.Using the optimal(9Cho+3Cr)/4Ci threshold of 0.73 in the peripheral zone,the sensitivity and specificity in detecting cancer were 93.3%and 99.1%.Conclusion(9Cho+3Cr)/4Ci ratio is of higher diagnostic value for prostate cancer,and the value of(9Cho+3Cr)/4Ci in the peripheral band is better than the central belt.
