摘  要:目的探讨LSP1 rs3817198位点多态性、常见风险因素与乳腺癌的相关性。方法选取2013年7月至2018年1月,青岛市市立医院就诊患者,完善所有参与试验患者临床资料并采集外周血,提取DNA,对位点基因进行分型。采用病例-对照研究,对105例乳腺癌患者和382例健康妇女进行研究,分析年龄等相关因素对乳腺癌患病风险的影响及LSP1 rs3817198的基因型分布,采用logistic回归分析基因位点与乳腺癌患病风险之间的关系。结果观察组患者初潮年龄小于对照组,差异有统计学意义(P=0.02)。观察组中无母乳喂养史者、未生育史者与乳腺癌发病相关(P=0.01)。观察组与对照组间LSP1 rs3817198的基因型频率分布差异有统计学意义(P=0.02)。变异基因型可增加乳腺癌患病风险,基因型TC(OR=2.02,95%CI=0.91~4.47),CC(OR=2.68,95%CI=1.32~5.46)。乳腺癌致病危险因素的分层分析显示初潮早、未绝经、有肿瘤家族病史妇女中携带TC、CC基因型者乳腺癌患病风险增加,有哺乳史则为保护性因素。结论LSP1 rs3817198基因的遗传多态性可增加女性罹患乳腺癌的危险性,其可作为检测妇女乳腺癌易感性的重要生物标志物。Objective To explore the correlation between rs3817198 polymorphism of LSP1 gene,relative risk factor and breast cancer.Methods 105 patients with breast cancer admitted to Qingdao Municipal Hospital from July 2013 to January 2018 were enrolled.382 healthy women were also enrolled as study controls.Blood samples were collected for DNA extraction and the genotype of rs3817198 of LPS1 gene was determined by real-time PCR and sequencing.Conditional logistic regression model was used for the correlation analysis.Results The age of first menstruation of women with breast cancer was much younger than that of controls(P=0.02).Women with no breastfeeding or childbearing experiences were more likely to have breast cancer(P=0.01).The difference of LSP1 rs3817198 genotype frequency distribution was statistically significant between women in two groups(P=0.02).Our results showed that the mutant genotype could increase the risks of breast cancer,especially TC genotype(OR=2.02,95%CI:0.91-4.47)and CC genotype(OR=2.68,95%CI:1.32-5.46).Stratification analysis of risk factors for breast cancer showed that having TC or CC genotype,younger age of first menstruation,family history of cancer and premenopause were risk factors of breast cancer,while having breastfeeding experience was a protective factor of breast cancer.Conclusions LSP1 rs3817198 polymorphism is correlated with breast cancer,which may be used as a biomarker in the screening of breast cancer.
