摘　　要：目的通过综合措施改善口腔环境,主动预防和降低上消化道肿瘤术后行放化疗患者口腔溃疡的发生。方法选择我院收治的意识清醒的上消化道肿瘤术后已行首次放化疗而未发生口腔溃疡的患者186例,随机分为对照组和预防组。对照组采用传统的复方硼酸含漱液含漱,预防组同时采用复方硼酸含漱、口服蒙脱石散(思密达)、纠正口腔pH值、牙线清洁口腔残渣等综合方法,观察并比较两组患者第二次放化疗结束后口腔溃疡的发生率。结果预防组口腔溃疡发生率(25.8%)显著低于对照组(66.7%)(P<0.01)。结论本研究通过综合措施改善口腔环境,有效地预防和降低了上消化道肿瘤术后放化疗患者口腔溃疡的发生,预防方法简便、实用、有效。Objective To prevent and reduce the occurrence of oral ulcers in patients with upper gastrointestinal cancer who had radiotherapy and chemotherapy after operation.Methods A total of 186 conscious patients were selected who had undergone upper gastrointestinal cancer surgery,and had no oral ulcers until enrollment after their first radiotherapy and chemotherapy.They were randomly divided into the control group and the prevention group.Patients in control group were gargled only with compound boric acid gargle.At the same time,the prevention group got comprehensive measures including compound boric acid gargle,oral montmorillonite,correction of oral PH value,floss for cleaning oral residue and so on.The incidence of oral ulcer was observed after the second dose of radiotherapy and chemotherapy.Results The incidence of oral ulcer in the control group was 66.7%.And the incidence of oral ulcer was 25.8%in the prevention group.It was significantly lower in prevention group than in control group(P<0.01).Conclusion The comprehensive measures to improve oral environment can effectively prevent and reduce the incidence of oral ulcer in patients with upper gastrointestinal cancer who had radiotherapy and chemotherapy after operation.These prevention methods are simple,practical and effective.
