摘 要:目的探讨液基细胞学(LBC)联合B-Raf原癌基因(BRAF)V600E基因检测在甲状腺乳头状癌诊断中的应用价值。方法90例甲状腺结节患者术前均进行了细针穿刺细胞学检查(FNAC)活检,穿刺标本的涂片分别采用传统涂片(CS)和LBC制片,同时对标本进行BRAF V600E基因检测。以术后组织病理学诊断结果为金标准,分析CS、LBC单独及LBC联合BRAF V600E基因检测对甲状腺乳头状癌中的诊断价值。结果40例患者为甲状腺乳头状癌,CS检测甲状腺乳头状癌的灵敏度、特异度、准确度分别为72.50%、96.00%、85.56%;LBC检测甲状腺乳头状癌的灵敏度、特异度、准确度分别为85.00%、98.00%、92.22%;BRAF V600E基因检测甲状腺乳头状癌的灵敏度、特异度、准确度分别为67.50%、100%、85.56%;LBC联合BRAF V600E基因检测甲状腺乳头状癌的灵敏度、特异度、准确度分别为95.00%、98.00%、96.67%。结论LBC制片能有效提高涂片质量,联合BRAF V600E基因检测可提高术前甲状腺乳头状癌的诊断准确度。Objective To explore the value of liquid-based cytology(LBC)combined with B-Raf proto-oncogene(BRAF)V600E gene detection in the diagnosis of papillary carcinoma of thyroid.Method Fine needle aspiration cytology(FNAC)were collected preoperatively from 90 patients with thyroid nodules.The biopsy specimens were prepared by conventional smear(CS)and LBC technique respectively,and BRAF V600E gene test was performed for the specimens.With postoperative histopathological findings as the gold standard,the diagnostic value of CS,LBC and LBC combined with BRAF V600E gene detection for papillary carcinoma of thyroid were determined and compared.Result Forty patients were identified as papillary carcinoma of thyroid;the sensitivity,specificity and accuracy of CS in detecting papillary carcinoma of thyroid were 72.50%,96.00%and 85.56%;for LBC in papillary carcinoma of thyroid diagnosis,the corresponding values were 85.00%,98.00%and 92.22%;as to BRAF V600E gene detection,the sensitivity,specificity and accuracy in detecting papillary carcinoma of thyroid were 67.50%,100%,and 85.56%;notably,the results for LBC combined with BRAF V600E gene detection achieved 95.00%,98.00%and 96.67%,respectively.Conclusion LBC technique can effectively improve the quality of smear specimens,furthermore,the addition of BRAF V600E gene detection would result in improved the preoperative diagnostic accuracy for thyroid papillary carcinoma.
