摘　　要：目的探讨经尿道双极等离子体电切术治疗膀胱癌的临床疗效及对患者生活质量的影响。方法采用随机数字表法将128例膀胱癌患者分为对照组和研究组,每组64例,对照组患者采用膀胱部分切除术治疗,研究组患者采用经尿道双极等离子体电切术治疗。比较两组患者的围手术期指标(手术时间、术中出血量、留置导尿管时间和住院时间)、并发症发生率、生活质量及术后复发率。结果研究组患者的手术时间和住院时间均明显短于对照组,术中出血量明显少于对照组,差异均有统计学意义(P﹤0.01)。研究组患者的并发症总发生率低于对照组,差异有统计学意义(P﹤0.05)。治疗前,两组患者的认知、行为、身体功能、社会功能评分比较,差异均无统计学意义(P﹥0.05);治疗后,两组患者的认知、行为、身体功能、社会功能评分均高于本组治疗前,且研究组患者的上述评分均高于对照组,差异均有统计学意义(P﹤0.05)。术后3个月、6个月、1年、2年,研究组患者的复发率均低于对照组,差异均有统计学意义(P﹤0.05)。结论经尿道双极等离子体电切术治疗膀胱癌的临床疗效较好,且可提高患者的生活质量,值得在临床中推广应用。Objective To investigate the clinical effect of bipolar plasmakinetic transurethral resection for bladder cancer and the effect on patients’quality of life.Method A random number table was used to randomize 128 patients with bladder cancer to control group and study group,respectively,with 64 cases in each.The control group was treated with partial cystectomy,while those in the study group were administered with bipolar plasmakinetic transurethral resection.Perioperative indicators(operative time,intraoperative blood loss,time of indwelling catheter,and length of hospital stay),incidence of complications,quality of life and postoperative recurrent rate were compared between the two groups.Result The operative time and hospitalization time in study group were significantly shorter than those in the control group,meanwhile,the volume of intraoperative blood loss was significantly less than that of the control group,and the differences were statistically significant(P<0.01).The incidence of complications in the study group was significantly lower compared to control group(P<0.05).Before treatment,the cognitive,behavioral,physical fumction,and social function scores of the two groups were similar(P>0.05),while were all increased after treatment as compared with those before treatment,furthermore,the scores in study group were evidently higher than those of the control group,showing statistically significant difference(P<0.05).In 3 months,6 months,1 year,and 2 years after operation,the recurrence rate of the study group were all lower than that of the control group(P<0.05).Conclusion Bipolar plasmakinetic transurethral resection is clinically effective in the treatment of bladder cancer,and may improve patients’quality of life.
