开腹手术史对腹腔镜结肠癌根治术后并发症的影响分析

摘　　要：目的探讨开腹手术史对腹腔镜结肠癌根治术后并发症的影响。方法选取2017年1月至2019年1月荆门市第一人民医院行腹腔镜结肠癌根治手术的有开腹手术史的患者100例和无开腹手术史的患者100例进行比较分析。观察两组患者在吻合口出血、吻合口瘘、术后肠梗阻、切口感染、腹腔感染、术后尿潴留、腹腔淋巴漏、吻合口狭窄、肺部感染、心脑血管意外等相关并发症。结果两组患者术后并发症发生率比较,差异无统计学意义(P>0.05)。结论开腹手术史对于腹腔镜结肠癌根治手术患者术后并发症发生率无明显的影响,因此对于有开腹手术史的患者进行腹腔镜结肠癌根治手术是安全可行的。Objective To investigate the influence of laparoscopic operation history on postoperative complications of laparoscopic radical resection for colon cancer.Methods A retrospective analysis was made on 200 patients who underwent laparoscopic radical operation for colon cancer in our hospital from January 2017 to January 2019,100 patients had a history of laparotomy and 100 patients without a history of laparotomy.The complications,including anastomotic bleeding,anastomotic fistula,intestinal obstruction,incision infection,abdominal infection,urinary retention,abdominal lymphatic leakage,anastomotic stenosis,pulmonary infection,and cardio-cerebrovascular accident,were observed.Results There was no significant difference between patients with history of laparotomy and those without history of laparoscopic radical colectomy(P>0.05).Conclusion The history of laparotomy has no obvious influence on postoperative complications of laparoscopic radical resection for colon cancer patients,so laparoscopic colon cancer radical operation for patients with history of laparotomy is safe and feasible.
