胰腺实性-假乳头状瘤9例病理分析

摘　　要：目的对胰腺实性-假乳头状瘤进行病理分析,并探讨鉴别诊断方法。方法选取肇东市人民医院2014年1月至2018年12月间收治的9例胰腺实性-假乳头状瘤患者,分析患者病理特征与免疫组织化学染色情况。结果肉眼可见肿物直径在1.2~8 cm,平均直径(4.2±0.8)cm。9例患者包括6例完整包膜、3例部分包膜;2例囊性、3例囊实性及4例实性。肿瘤细胞的β-Catenin、NSE、CK与Vimentin弥漫性阳性,但CgA阳性率低;且Ki-67指数为0.05%~1.50%。结论临床需掌握胰腺实性-假乳头状瘤的组织学检查与免疫组化特点并与胰腺血管活性肽瘤、胰腺浆液性、黏液性囊腺癌及胰母细胞肿瘤相鉴别诊断。Objective To analyze the pathological features of pancreas solid-pseudopapilloma and to explore its differential diagnosis method.Methods Nine patients with pancreas solid-pseudopapilloma admitted to People's Hospital of Zhaodong City from January 2014 to December 2018 were selected as the subjects.The pathological features and immunohistochemical staining were analyzed.Results The diameter of the tumor was 1.2 to 8 cm and the average diameter was(4.2±0.8)cm.Among the 9 patients,there were 6 complete capsule,3 partial capsule;2 cystic,3 solid-cystic and 4 solid.Tumor cells were diffusely positive forβ-Catenin,NSE,CK and Vimentin,while the positive rate of CgA was low,and the Ki-67 index was 0.05%to 1.50%.Conclusion The histological examination and immunohistochemical features of pancreas solid-pseudopapilloma should be mastered in clinic and be differentiated from pancreatic vasoactive peptide tumor,pancreatic serous,mucinous cystadenocarcinoma and pancreatic blastocytic tumor.
